摘　　要：目的:探讨中药五类新药(丁桂油软胶囊)治疗肠易激综合征(irritable bowel syndrome,IBS)的临床疗效.方法:随机抽取2012-01/2015-03上海市第七人民医院消化内科收治的78例IBS患者的临床资料.按照随机数字表法将患者分为A组、B组和C组,每组例数为26例.A组患者采取低剂量丁桂油软胶囊治疗.B组患者采取高剂量丁桂油软胶囊治疗.C组患者采取丁桂油软胶囊安慰剂治疗.观察3组患者临床疗效、中医症状疗效、腹痛疗效、疼痛起效时间、疼痛缓解时间、症状总积分、中医症状积分、不良反应发生情况.观察患者降钙素基因相关肽(calcitonin gene-related peptide,CGRP)、P物质(P substance,SP)、胆囊收缩素(cholecystokinin,CCK)变化情况.结果:A、B组临床总有效率、中医症状总有效率、腹痛总有效率显著高于C组(P<0.05).B组、C组疼痛起效时间显著优于A组(P<0.05).3组患者疼痛缓解时间比较,差异无统计学意义.治疗后,B组症状总积分、中医症状积分均优于A组和C组(P<0.05).3组患者均未发生严重不良反应事件.治疗后,B组CGRP、SP、CCK阳性表达面积、平均光密度值、聚合光密度值均优于A组和C组(P<0.05).结论:丁桂油软胶囊治疗肠易激综合征临床疗效佳,能够显著改善患者中医症状总疗效、腹痛总疗效、症状总积分、中医症状积分,具有较高的临床推广价值.AIM：To evaluate the clinical effects of Dingguiyou capsule in irritable bowel syndrome（IBS）.METHODS：Seventy-eight patients with IBS were randomly and equally divided into three groups：A,B and C.Group A was treated with low-dose Dingguiyou capsule,group B was treated with high-dose Dingguiyou capsule,and group C was treated with a placebo.The clinical effects,efficacy on relief of traditional Chinese medicine（TCM） syndrome,efficacy on relief of abdominal pain,time to pain reduction,time to pain relief,total symptom score,TCM symptom score,and adverse reactions were compared for the three groups.The levels of calcitonin generelated peptide（CGRP）,P substance（SP）,and cholecystokinin（CCK） were also compared.RESULTS：The clinical effects,efficacy on relief of TCM syndrome,and efficacy on relief of abdominal were significantly better in groups A and B than in group C（P〈0.05）.The time to pain reduction was longer in groups B and C than in group A（P〈0.05）.There was no significant difference in the time to pain relief for the three groups（P〈0.05）.The total symptom score and TCM symptom score were significantly higher in group B than in groups A and C（P〈0.05）.There were no serious adverse events.The levels of CGRP,SP,and CCK after treatment were better in group B than in groups A and C.CONCLUSION：Dingguiyou capsule has good clinical effects for patients with IBS,and it can improve the efficacy on relief of TCM syndrome,the efficacy on relief of abdominal pain,total symptom score,and TCM symptom score.
