摘　　要：目的观察维格列汀对2型糖尿病合并冠心病患者血清脂联素、抵抗素水平的影响。方法将60例冠心病合并糖尿病患者随机分为2组,观察组给予维格列汀口服治疗,对照组给予二甲双胍口服治疗,2组均治疗10周。检测2组治疗前后血清脂联素(APN)、抵抗素(RET)、肿瘤坏死因子-α(TNF-α)、空腹血糖(FBG)和胰岛素(FINS)、总胆固醇(TC)、三酰甘油(TG)、游离脂肪酸(FFA)水平,计算胰岛素敏感指数(ISI)和胰岛素抵抗指数(HOMA-IR)。结果治疗后对照组FBG、APN、RET、FFA均较治疗前明显改善(P均<0.05);观察组除TC、TG无明显变化外,其余指标均较治疗前明显改善(P均<0.05),且观察组改善情况明显优于对照组(P均<0.05)。结论维格列汀能有效降低2型糖尿病合并冠心病患者血清RET水平,提高APN水平,改善胰岛素抵抗。Objective It is to explore the effect of Vildagliptin on the adiponectin and resistin of patients with 2 - diabetes mellitus complicating with coronary heart disease. Methods Sixty patients with coronary heart and 2 - diabetes mellitus were divided into observation group and the control group randomly. The control group was treated by metformin orally and the obser- vation group was treated by vidagliptin orally. The level of APN, RET, TNF- a, FBG, FINS, TC, TG, FFA were deter- mined before and after treatment, and the index of ISI and HOMA - IR were calculated. Results After treatment, the levels of FBG, APN, RET and FFA were improved significantly compared with that before treatment （ P all 〈 0.05 ） , all the indexes except TC, TG were improved significantly compared with that before treatment （ P all 〈 0.05） in observation group, and the improvements were better than that in the control group （ P all 〈 0.05 ）. Conclusion Vildagliptin could significantly reduce the level of RET, increase the level of APN and improve the insulin resistance of patients with 2 - diabetes mellitus with corinary heart disease.
