妊娠期糖尿病新诊断标准及孕期管理对妊娠结局的影响

摘　　要：目的采用新妊娠期糖尿病诊断标准一步法及孕期管理,加强母儿监护后,分析妊娠期糖尿病的妊娠结局。方法回顾性分析2013年10月~2014年9月在上海交通大学医学院附属瑞金医院妇产科分娩的120例妊娠期糖尿病孕妇（Ⅰ组）与同期120例健康孕妇（Ⅱ组）的妊娠结局。结果采用妊娠期糖尿病新诊断标准后,分娩人群中妊娠期糖尿病孕妇占9.14%,经过孕期健康管理,应用胰岛素的孕妇占妊娠期糖尿病孕妇的8.33%。Ⅰ、Ⅱ两组羊水过多、产后出血、新生儿窒息、巨大儿、早产、新生儿肺炎差异无统计学意义（P〉0.05）;胎膜早破差异有统计学意义（P〈0.05）。结论妊娠期糖尿病采用新的诊断标准,结合孕期管理,母儿并发症得到有效的控制。Objective To analyze the gestational perinatal outcome of gestational diabetes mellitus using the new diagnostic criteria and administrant gestational, and strengthen the work of maternal and infant care. Methods 120 GDM pregnant women（study group I ）and 120 pregnant women（control group H ）who delivered in Ruijin Hospital during October 2013 and September 2014 were enrolled into this study. The gestational outcomes. Results The percentage of GDM patients was 9.14% by the new GDM diagnostic criteria. After the clinic health care, the ＂ percentage of insulin use was 8133% in GDM pregnant women, There were no significant differences of incidence of Polyhydramnios, postpartum hemorrhage, neonatal asphyxia, fetal macrosomia, preterm labor, neonatal pneumonia between the study group and the control group（P 〉 0.05）.While there were significant differences of incidence of premature ruloture of membrane between the two groups （P 〈 0.05）. Conclusion The gestational complications can be controlled appropriately when using the new diagnostic criteria for gestational diabetes mellitus, combined with administrant gestational.
