摘　　要：选择性围肝门部切除可以满足肝门部胆管癌根治性手术切除的需要。我们设计了肝部分Ⅴ段、Ⅳb段及尾状叶整体切除术式,切除标本类似哑铃状,因此命名为"哑铃式"肝切除术,其手术指征为:(1)BismuthⅡ型肝门部胆管癌,以及部分肿瘤局限于一级肝管内的Ⅲa、Ⅲb型肝门部胆管癌;(2)无门静脉分叉部或左右支受侵;(3)第3站淋巴结无转移;(4)无肝内或远处组织器官转移。本式具备术后恢复快、并发症少等特点,尤其适用于老龄患者。Selective perihilar resection could meet the requirement of radical resection of hilar cholangiocarcinoma. We designed a surgical procedudre to resect segment Ⅰ ,Ⅳ b and partial V. As the reseeted tissue resembles a dumbbell, this surgical technique is named dumbbell type radical resection. The indications of this procedure include： （ 1 ） hilar cholangiocarcinoma, Bithmuth Ⅱ and Ⅲ with the tumor located in the left or right hepatic ducts ; （2） no portal invasion; （ 3 ） no metastasis of the third station lymph node; and （4） no liver or distant organ metastasis. We considered that the procedure characterized by rapid postoperative recovery and fewer complications is especially suitable for elderly patients.
