探讨联合检测血清Cys-C和RBP对慢性肾脏病早期肾功能诊断的意义

摘　　要：目的探讨联合检测血清胱抑素C(Cys-C)和视黄醇结合蛋白(RBP)水平在慢性肾脏病(CKD)早期肾功能损伤中的诊断价值。方法收集105例CKD患者根据肾小球滤过率(GFR)水平分成的3组,在日立7600全自动生化分析仪上测定3组血清样本的Cys-C、RBP、肌酐(SCr)、尿素氮(Urea)水平,并与39例健康体检者(正常对照组)的血清样本检出结果进行相关统计学分析。结果血清Cys-C和RBP在CKD各组中显著高于正常对照组,比较差异有统计学意义或高度统计学意义(P<0.05或P<0.01);CKD各组Cys-C和RBP阳性检出率明显高于SCr和Urea,比较差异有统计学意义或高度统计学意义(P<0.05或P<0.01);Ⅰ组的Cys-C和RBP单独检测与联合检测比较,差异无统计学意义(P>0.05);Ⅱ组和Ⅲ组的Cys-C和RBP单独检测与联合检测比较,差异有统计学意义(P<0.05)。结论血清Cys-C和RBP是反映GFR理想的内源性指标,联合检测血清Cys-C和RBP水平有助于反映CKD早期肾脏功能进展,对CKD各种肾小球滤过功能损伤早期诊断、病情进展、严重程度、疗效观察及预后等方面有着重要的临床应用价值。Objective Testing of serum Cystatin C（Cys-C）and retinol binding protein（RBP）level with chronic kidney disease（CKD）of early renal injury diagnosis in patients.Methods 105 cases of CKD patients hospitalized were divided into 3groups,3groups serum samples of Cys-C,RBP,creatinine（SCr）,urea nitrogen（Urea）level on the 7600 automatic biochemical analyzer,,and 39 healthy subjects（normal control group）serum samples testing results were statistical analysis.Results Serum Cys-C and RBP were significantly higher than those in the CKD group in the normal control group,there were statistically significant or highly significant difference（P〈0.05 or P〈0.01）;CKD group of Cys-C and positive rate of RBP was significantly higher than that of SCr and Urea,were statistically significant or highly significant difference（P〈0.05 or P〉0.01）;comparison of Cys-C and RBP single test and combined testing of I group,no significant difference（P〈0.05）;a comparison between Cys-C and RBP single test and combined testing of group II and III group,the difference was statistically significant（P〈0.05）.Conclusion The serum levels of Cys-C and RBP is a reflection of endogenous index GFR ideal,combined testing of serum Cys-C and RBP levels contribute to the progress of CKD which could reflect early renal function,has important clinical application value of CKD in various glomerular filtration function injury early diagnosis,disease progression,severity,curative effect and prognosis.
