应用响应曲面模型法改善肿瘤患者术后静脉镇痛效果的临床观察

摘  要:目的应用响应曲面分析法建立最佳的舒芬太尼复合氟比洛芬酯静脉术后镇痛模型,获得满意的镇痛效果,降低副作用的发生。方法选择ASAI-II级的肿瘤患者,年龄18-70岁,行胸腹部手术70例,随机分为对照组和实验组。对照组术后给予静脉镇痛泵舒芬太尼200μg+氟比洛芬酯150mg+生理盐水到250ml,实验组通过建立舒芬太尼与氟比洛芬酯相互作用响应曲面模型,获得理想的药物配置浓度,与常规配比方案进行效果和副作用发生率的比较。结果通过应用响应曲面分析法获得的舒芬太尼复合凯纷镇痛浓度方案,镇痛效果与常规配比方案没有统计学差异,恶心呕吐、头晕和尿潴留的发生率低于对照组。结论应用响应曲面分析法可以有效地分析两种药物配比浓度,有利于获得理想的镇痛效果,减少副反应的发生。Objective Response surface analysis was applied to describe the pharmacodynamic interactions between sufentanil and flurbiprofen in postoperative analgesia to get a better analgesic effect,and less side effects.MethodsTumor patients,ASA I-II,aged 18 to 70,70 cases of thoracic or abdominal operation,were randomly divided into control group and experimental group.The control group was given intravenous postoperative analgesia pump sufentanil200μg +flurbiprofen 150mg+0.9%saline to 250 ml,the experimental group through the establishment of sufentanil interact with flurbiprofen response surface model,obtain ideal drug concentration,and the comparison to conventional schemes of effect and the incidence of side effects.Results By applying the response surface analysis of sufentanil compound flurbiprofen analgesia,analgesic effect has no statistical difference compairied with conventional scheme,the incidence of nausea and vomiting,dizziness,and urinary retention is lower than control group.Conclusion The application of response surface analysis method can effectively analyse two drugs concentration ratio,obtain an ideal analgesic effect,reduce the occurrence of adverse events.
