摘  要:目的探讨经阴道三维彩超(3DTVU)联合超声造影(CEUS)在盆腔静脉瘀血综合征(PCS)中的诊断价值。方法对我院2012年7月-2015年1月92例临床疑似PCS的患者,经腹腔镜手术证实87例,先行3DTVU,对宫旁、卵巢周围的盆腔静脉进行三维彩色重建,后行CEUS,采用CEUS定量分析技术,测量盆腔静脉的半排时间,并与腹腔镜美兰实验结果对照。结果 87例PCS患者,3DTVU诊断符合率82%(76/92),敏感度83%(77/87),特异度60%(3/5);CEUS定量分析以半排时间为125s做为CUTOFF值,其诊断符合率88%(81/92),敏感度88%(77/87),特异度80%(4/5);3DTVU联合CEUS定量分析诊断符合率93%(86/92),敏感度94%(82/87),特异度80%(4/5)。3DTVU联合CEUS较单独应用3DTVU及CEUS均有统计学意义(P<0.05)。结论经阴道三维彩超联合超声造影定量技术对盆腔静脉瘀血综合征诊断有较好的诊断价值。Objective To explore the value of 3 dimentional transvaginal ultrasound(3DTVU)and contrast ultrasound examination(CEUS)in diagnosis of pelvic venous congestion syndrome(PCS).Methods 3DTVU were examined in 87 cases of PCS which were confirmed by laparoscopy,then CEUS were proformed which were focus on the pelvic vein,recorded the half-reducing time and compared with the laparoscopy.Results Among the 87 cases of PCS,the diagnostic accordance rate of 3DTVU was 82%(76/92),sensitivity was 83%,specificity was 60%;If we used the half-reducing time 125 sas the CEUS CUTOFF of quantitative analysis,the diagnostic accordance rate was 88%(81/92),sensitivity was 88%,specificity was 80%;The diagnostic accordance rate of 3DTVU CEUS were 93.1%(82/87),sensitivity was 94%,specificity was 80%.There were statistic differences between 3DTVU CEUS and 3DTVU or CEUS(P0.05).Conclusion 3DTVU CEUS have better diagnostic value in diagnosis of PCS
