缺血修饰白蛋白在缺血性和非缺血性胸痛鉴别诊断中的应用

摘　　要：目的:探讨缺血修饰白蛋白(IMA)在缺血性和非缺血性胸痛患者中的鉴别诊断作用。方法:选择我院急诊入院的胸痛患者257例,其中确诊为不稳定性心绞痛136例(缺血性胸痛组),121例可疑不稳定性心绞痛但后续排除诊断为非缺血性胸痛(非缺血性胸痛组)。每例患者都进行心电图(ECG)、心肌肌钙蛋白I(cTnI)、IMA水平测定。利用SPSS分析其相关性和ROC曲线下面积。结果:缺血性胸痛组IMA值高于对照组(P<0.001),胸痛组IMA敏感度和ROC曲线下面积均高于cTnI和ECG。IMA、ECG、cTnI联合检测的敏感度和ROC曲线下面积高于单独检测。结论:IMA在缺血性和非缺血性胸痛的鉴别诊断中有重要的应用价值。Objective：To investigate the application of ischemia-modified albumin in differential diagnosis of transient myocardial ischemia from non ischemia chest pain.Method：Using automatic biochemical analyzer and autoimmune electrochemical luminescence analyzer analysis the levels of IMA,cTnI in 257 cases of ischemic chest pain and non-ischemic chest pain,all the cases perform ECG.Using SPSS17.0analysis the correlation and the area under the ROC.Result：The levels of IMA,cTnI of serum in ischemic chest pain group were higher than those in non-ischemic chest pain group（P〈0.01）.The sensitivity and the area under the ROC of IMA all was higher than cTnI and ECG（P〈0.05）.The sensitivity was 94% and specificity was 70% when IMA,ECG and cTnI were combined detection.Conclusion：IMA has important clinical value in distinguishing ischemic chest pain and non-ischemic chest pain.
