三维适形放疗联合两种化疗方案治疗中晚期食管癌的临床疗效评价

摘　　要：目的：观察三维适形放疗联合同期多西紫杉醇联合顺铂、5－氟尿嘧啶联合顺铂两种化疗方案治疗中晚期食管癌临床疗效。方法选取临汾市第四人民医院收治的中晚期食管癌患者86例作为研究对象，经临床病理证实，均行三维适形放疗，采用随机对照表将患者分为DC组和DF组，各43例，DC组为多西紫杉醇联合顺铂化疗方案，DF组为5－氟尿嘧啶联合顺铂化疗方案，观察两组缓解率、不良反应等，随访2年，观察患者的病死率。结果 DC组治疗有效率为81．40％，与DF组（76．74％）比较差异未见统计学意义（ P＞0．05）。 DC组白细胞减少的发生率为69．77％，较DF组高，胃肠道反应的发生率为30．23％，较DF组低，差异有统计学意义（ P＜0．05）。 DC组病死率为44．19％，较DF组低，但差异未见统计学意义（ P＞0．05）。结论两种化疗联合同步放疗方案近期疗效差异未见统计学意义，但DC组胃肠道反应发生率相对较低，可提高患者的耐受性。Objective To observe the clinical efficacy of three dimensional conformal radiotherapy combined with concurrent docetaxel combined with cisplatin, 5-fluorouracil and cisplatin two chemotherapy regimen on esophageal carcino-ma.Methods Chosed 86 cases of advanced esophageal cancer patients treated in the fourth people’ s hospital of Linfen as the research objects, proved by pathology, they were performed in three dimensional conformal radiotherapy, the 86 cases of advanced esophageal cancer patients were divided into DC group and DF group with random comparison table methods, 43 cases in each group, DC group for the combination of docetaxel and cisplatin chemotherapy, DF group with 5-fluoroura-cil and cisplatin chemotherapy scheme, the response rate, toxicity of the two groups followed up for 2 years, and the fatality rate of patients was observed.Results In DC group, the effective rate （ 81.40%） compared with the DF group （76.74%）, the difference was not significant（P〉0.05）.White blood cells reduce rate of DC group（69.77%）was high-er than that of DF group, the gastrointestinal tract incidence rate（30.23%） was lower than that of DF group, there were significant differences （P〈0.05）.The mortality（44.19%） in DC group was lower than that of DF group, but there was on significant difference（P〉0.05）.Conclusions No significant differences in curative effect between the two chemother-apy combined with concurrent radiotherapy plan, but gastrointestinal tract reaction rate is relatively lower in DC group, and can enhance the tolerance of patients.
