CT主动脉心电门控成像中不同对比剂注射方案与对比剂剂量的相关性研究

摘　　要：目的比较CT主动脉心电门控成像中两种不同对比剂注射方案对对比剂剂量的影响。方法搜集临床上因怀疑主动脉血管性疾病行CT血管成像(CTA)者40例,随机分为A、B两组,A组平均体重(69.08±11.45)kg,平均体质量指数(BMI)(25.91±4.5)kg/m2;B组平均体重(65.34±12.65)kg,平均BMI(24.57±4.1)kg/m2。对比剂(350 mg I/ml)注射前置于37℃恒温箱预热,使用双筒高压注射器经肘静脉留置针(20 G)注入。检查前先注射20 ml生理盐水,注射流率5.0 ml/s,检查注射通道是否通畅。A组注射流率4 ml/s,对比剂用量1.0 ml/kg。B组第一期以5 ml/s注射20 ml,第二期以3 ml/s注射剩余量;B组对比剂总量=1.0 ml/kg×体重-4 ml/s×(B方案对比剂注射时间-A方案对比剂注射时间)。两组注射对比剂后以3 ml/s流率无缝隙注射生理盐水50 ml。两组均选择回顾性心电门控扫描模式,采用智能触发技术确定扫描延迟时间,监测平面为主动脉弓降部,ROI阈值≥150 HU。采用独立样本t检验比较A、B两组图像质量评分、主动脉平均增强后CT值及对比剂用量。结果 A组和B组主动脉平均增强后CT值分别为(267.4±21.6)HU、(257.7±27.0)HU,主动脉及其分支的图像质量评分分别为3.85±0.37、3.75±0.55,差异均无统计学意义(P>0.05);A组和B组对比剂剂量分别为(69.08±11.45)ml和(57.6±9.34)ml,差异有统计学意义(P<0.05)。结论对比剂注射流率5 ml/s+3 ml/s分期注射可在不影响图像质量的前提下降低主动脉CTA的对比剂用量。Objective To compare the effects of different contrast medium injection on the contrast media dose in 256-slice spiral CT ECG-gated aortic angiography. Methods 40 cases of patients who were suspected with aortic diseases and had undergone aortic angiography were divided into two groups randomly,the contrast media dose was 350 mg I / ml.,the middle weight is（ 69. 08 ± 11. 45） kg and average BMI is（ 25. 91 ± 4. 5） kg / m2 in group A; the middle weight is（ 65. 34± 12. 65） kg average BMI is（ 24. 57 ± 4. 1） kg / m2 in group B. The contrast medium was preheated in 37℃ incubator before injection and injected through elbow vein indwelling needle by binocular high pressure injector. The 20 ml normal saline was injected to check injection gallery before checking,the injection speed was 5. 0 ml / s. The injection rate in group A was 4ml / s and total dose was 1. 0 ml / kg; in group B,the injection rate was 5 ml / s and total dose was 20 ml in the first step,and the others was injected with 3 ml / s in the second step. and the total dose in group B was equal to 1. 0 ml / kg multiply weight and substract（ 4 ml / s multiply the time difference in two groups）. The 50 ml normal saline was injected immediately after contrast medium injection was given in two groups,and injection rate was 3 ml / s. The retrospective heart-gated scanning model was used and intelligent touch scanning technique was used to determine scanning time in two groups,and the monitor plane was descending part of aortic arch,the ROI threshold value was ≥150 HU. The independent sample t test was used to compare the image quality score,the mean aortic CT value after enhancement and contrast medium dose.Results The mean aortic values after enhancement was（ 267. 4 ± 21. 6） HU、（ 257. 7 ± 27. 0） HU,the image quality score was 3. 85 ± 0. 37、3. 75 ± 0. 55 in two groups,there were no statistically significant difference; the contrast dose were（ 69.08 ± 11. 45） ml 和（ 57. 6 ± 9. 34） ml,there was statistically signi
