摘　　要：目的：探讨局灶性机化性肺炎（FOP）的 CT 表现。方法回顾性分析17例经手术或穿刺病理证实的 FOP 的 CT 表现,所有患者均行128层螺旋 CT 平扫,13例行多期增强扫描检查。结果5例位于左肺,12例位于右肺,11例位于下肺,5例位于上肺,1例位于中肺。14例位于胸膜下。9例病灶呈楔形团块影,5例呈结节影,3例呈球形肿块影。边界清楚5例,模糊8例,边界清楚或模糊4例。毛刺11例,弓形凹陷征2例,彗星尾征1例。病灶内可见坏死12例,空泡5例。胸膜肥厚10例,胸膜牵拉12例。增强扫描13例,其中8例病变内见肺血管影,延时强化11例,12例强化不均匀。合并肺门或纵隔淋巴结肿大4例。结论FOP 的CT 表现多样,缺乏特征性表现。影像表现为楔形团块合并一定张力的坏死区,相邻胸膜肥厚时需考虑本病。Objective To explore the CT findings of focal organizing pneumonia （FOP）.Methods CT findings of 1 7 patients with FOP confirmed by pathology were retrospective reciewed,All patients underwent CT plain scan and 13 patients underwent contrast-enhanced CT scan.Results The lesions were located in the left lung in 5 cases and the right lung in 12 cases;and in the lower lobe in 1 1 cases,the upper lobe in 5 cases and in the middle lobe 1 case;and in the peripheral lungs in 14 lesions.The lessions showed ir-regular mass in 9 cases,nodule mass in 5 cases,and spherical lumps in 3 cases.The lesions showed well-defined boundary in 5 ca-ses,fuzzy boundary in 8 cases,and well-defined or fuzzy boundary in 4 casese.The lesions had speculation in 1 1 cases,bow indenta-tion sign in 2 cases,and comet tailartifact in 1 case.Lessions showed necrosis in 12 cases and vacuoles in 5 cases.Pleural thickening was showed in 10 cases and pleural stretch in 12 cases.In 13 cases with contrast-enhanced CT scan,pulmonary vessels penetrated through the lesions were seen in 8 cases,with delayed enhancement in 1 1 cases and heterogenous enhancement in 12 cases.Hilar lymph node enlargement or mediastinal lymphadenectasis were revealed in 4 cases.Conclusion FOP shows diverse CT findings which are lack of specificity,while it should be taken into account when CT findings show wedge mass with a certain tension of nec-rosis and adjacent pleural thickening.
