肾脏移植受者肾脏功能衰竭的相关因素研究

摘　　要：目的探讨肾脏移植受者肾功能衰竭的影响因素。方法采集120例首次接受肾移植并且术后6个月内移植肾功能良好受者的临床资料,随访7~11年。采用多因素Logistic回归分析慢性移植肾功能衰竭的影响因素。结果随访期间,慢性移植肾功能衰竭发生24例。多因素Logistic回归分析,结果发现性别（O^R=2.238）、年龄（O^R=1.653）、急性排斥反应（O^R=3.102）、人类白细胞抗原错配（O^R=2.125）、病毒感染（O^R=3.679）、血清肌酐（O^R=1.286）、尿蛋白（O^R=3.996）、甘油三酯（O^R=2.867）、高血压（O^R=1.683）以及治疗依从性差（O^R=2.057）均是慢性移植肾功能衰竭的影响因素。结论对于男性、高龄、急性排斥反应次数多、人类白细胞抗原错配多、伴有病毒感染、高血压、治疗依从性差以及血清肌酐、尿蛋白、甘油三酯水平升高的肾移植受者易发生肾功能衰竭。【Objective】 To investigate the influence factors of chronic allograft failure in renal transplant recipients. 【Methods】 The clinical data of 120 recipients with first renal transplant and the normal renal function in postoperative 6 months were collected and they were followed up for 7 ~11 years. The Logistic regression analysis was used to analyze the influence factors on the chronic allograft failure. 【Results】 During the follow-up, 24 cases had chronic allograft failure. The results of Logistic regression analysis showed that gender（^OR = 2.238）, age（^OR = 1.653）, acute rejection（^OR = 3.102）, human leukocyte antigen mismatch（^OR =2.125）, virus infection（^OR = 3.679）, serum creatinine（^OR = 1.286）, urinary protein（^OR = 3.996）, triglyceride（^OR = 2.867）, hypertension（^OR = 1.683） and poor treatment compliance（^OR = 2.057） were the influence factors of chronic allograft failure in renal transplant recipients. 【Conclusion】 Renal transplant recipients who are male and older with a high frequency of acute rejection, the more mismatch of human leukocyte antigen,virus infection, hypertension, poor treatment compliance and the high levels of serum creatinine, urinary protein and triglyceride could be prone to have chronic allograft failure.
