妈富隆联合内美通预防卵巢巧克力囊肿术后复发的效果观察

摘　　要：目的观察妈富隆联合内美通预防卵巢巧克力囊肿术后复发的临床效果及安全性。方法将120例腹腔镜巧克力囊肿剔除术后患者按数字表法随机分成3组,每组40例,A组术后给予妈富隆口服,B组给予内美通口服,C组给予妈富隆联合内美通口服;术后随访12个月,观察3组患者的总有效率、复发率、妊娠率、卵巢功能改善情况及不良反应。结果术后6个月,C组患者总有效率为97.5%,显著高于A组的80.0%和B组的82.5%(P<0.05),C组复发率(2.5%)低于A组(20.0%)或B组(17.5%)(P<0.05);术前3组患者血清卵泡刺激素(FSH)、雌二醇(E2)、黄体生成素(LH)水平及窦状卵泡(Fo)数比较差异无统计学意义(P>0.05),术后1个月,C组患者血清E2、LH水平及Fo数高于A组和B组患者(P<0.05),血清FSH水平低于A组和B组患者(P<0.05)。术后12个月,C组患者妊娠率为57.5%,显著高于A组27.5%和B组30.0%(P<0.05)。C组不良反应发生率(2.5%)低于A组(15.0%)或B组(15.0%)(P<0.05)。结论腹腔镜下卵巢巧克力囊肿剔除术后给予妈富隆联合内美通能明显降低囊肿复发率,改善卵巢功能,提高妊娠率。Objective To explore the clinical efficacy and safety of marvelon combined with nemestran for the prevention of postoperative relapse in patients with chocolate cyst of ovary.Methods One hundred and twenty patients who had undergone laparoscopic ovarian chocolate cyst removal were randomly divided into Group A（n=40）,Group B（n=40） and Group C（n=40） by random number table. After operation,Group A was administered with orally marvelon,Group B orally nemestran,and Group C orally marvelon combined with nemestran. During 12 months of follow-up,the total efficacy rate,recurrence rate,pregnancy rate,improvement of ovarian function and side effects were observed among three groups.Results Six months after operation,the total efficacy rate in Group C was 97.5%,which was significantly higher than that in Group A（80.0%）or Group B（82.5%）（P〈0.05）,and the recurrence rate in Group C（2.5%） was lower than that in Group A（20.0%） or Group B （17.5%）（P〈0.05）.There were no significant differences in the levels of serum follicle-stimulating hormone（FSH）,estradiol（E2）, luteinizing hormone（LH） or the number of sinus follicle（Fo） among three groups before operation（P〉0.05）.One month after operation, Group C gained significantly higher serum E2 and LH levels and number of Fo,and significantly lower serum FSH level compared with Group A or Group B（P〈0.05）.The pregnancy rate in Group C was 57.5%during the 12 months of follow-up,which was significantly higher than that in Group A（27.5%） or Group B（30.0%）（P〈0.05）.The incidence of side effects in Group C（2.5%） was significantly lower than that in Group A（15.0%） or Group B（15.0%）（P〈0.05）.Conclusion For the patients who have undergone laparoscopic ovarian chocolate cyst removal, the postoperative administration of marvelon combined with nemestran can reduce the recurrence rate of the cyst,improve ovarian function and increase the pregnancy rate.
