摘 要:目的:研究彩色超声测量的慢性肾脏病患者肾脏皮质厚度、肾脏长度与肾小球滤过率的相关性,探讨应用彩色超声评估慢性肾脏病患者肾小球滤过率的可行性。方法:60例临床确诊为慢性肾脏病的非透析患者,使用彩色超声测量双肾长度和皮质厚度,应用Cockcroft-Gault(CG)公式计算肾小球滤过率估计值(estimated glomerular filtration rate,e GFR),分析慢性肾脏病患者肾脏皮质厚度、肾脏长度与肾小球滤过率的相关性。结果:肾脏皮质厚度为(6.1±1.2)mm,肾脏长度为(10.3±0.7)cm,e GFR为(35.8±21.5)ml/min。慢性肾脏病患者肾脏皮质厚度与肾小球滤过率呈正相关(r=0.523,P>0.05);肾脏长度与e GFR没有明显的相关性(r=0.044,P<0.05)。结论:彩色超声测量肾皮质厚度可以作为评估慢性肾脏病患者肾小球滤过率的一种较为简便易行的补充方法,为临床进一步检查和治疗提供建议。Objective To determine the correlation between the glomerular filtration rate(GFR) and the renal cortex thickness or the kidney length in chronic kidney disease patients, and to explore the feasibility of ultrasound for detecting the GFR. Methods Totally 60 nondialytic patients with chronic kidney diseases confirmed clinically were measured with color ultrasonograph for the lengths of both kidneys and the renal cortex thickness, and then Cockcroft-Gault(CG) formula was used to calculate the estimated glomerular filtration rate(e GFR) to analyze the correlation between the glomerular filtration rate(GFR) and the renal cortex thickness or the kidney length. Results It's shown that the renal cortex thickness was(6.1±1.2) mm, the kidney length was(10.3±0.7) cm and the e GFR was(35.8±21.5) ml/min. There was a positive correlation between the renal cortex thickness(r=0.523, P〈0.05), while no significant correlation between the kidney length and e GFR(r=0.044, P〈0.05). Conclusion Detection of the renal cortex thickness can be a supplementation for evaluating the GFR of the chronic kidney disease patients to provide references for further clinical examination and treatment.
