摘  要:目的探讨女性肛门尖锐湿疣(CA)组织中人乳头瘤病毒(HPV)感染的基因型及其临床意义。方法选取1985年1月-2015年5月江苏省苏州市吴江区中医院、南京市中医院、常州市中医院肛肠科手术切除的女性肛门及肛管CA石蜡组织标本150份,提取23种HPVDNA,采用基因扩增结合基因芯片技术对其组织进行23种HPV分型的检测,并对患者临床病理资料进行分析。结果150例女性肛门CA组织标本中检出HPV阳性者111例,HPV感染率为74.00%(111/150)。其中单一型HPV感染72例,阳性检出率为48.00%(72/150),多型HPV感染39例,阳性检出率为26.00%(39/150)。单一型HPV感染中HPV11型34例,阳性检出率为22.67%(34/150),HPV11型是最主要的感染型别;HPV6型29例,阳性检出率为19.33%(29/150)。多型HPV感染中,HPV6+11型13例,占多型感染的33.33%(13/39),是多型感染的主要型别;HPV11+18型5例,占多型感染的12.82%(5/39)。结论HPV6型、11型、6+11型、11+18型感染是女性肛门CA最主要的型别。基因扩增结合基因芯片技术是适合临床应用于HPV分型检测的诊断方法,其敏感性和特异性较好,适合分子流行病学的研究。Objective To explore the genotypes of human papillomavirus (HPV) infection in female anal condyloma acuminate and the clinical significance. Methods One hundred and fifty paraffin-embedded female anal and anal tube condyloma acuminate tissue specimens excised after surgery in Department of Proctology in Wujiang Hospital of Traditional Chinese Medicine, Nanjing Hospital of Traditional Chinese Medicine, and Changzhou Hospital of Traditional Chinese Medicine were selected, 23 kinds of HPV DNA were abstracted, gene amplification combined with gene chip technology were used to detect 23 kinds of HPV genotypes, the clinicopathological data was analyzed. Results Among 150 women, 111 women were found with positive HPV, HPV infection rate was 74. 00% (111/150) . Among 111 women with positive HPV, 72 women were found with single HPV infection (48.00%, 72/150), and 39 women were found With multiple HPV infection (26. 00% , 39/150) . Among the women with single HPV infection, HPV 11 was the most common HPV genotype, the positive detection rate was 22. 67% (34/150) ; 29 women were found with HPV 6, the positive detection rate was 19. 33% (29/150) . Among the women with multiple HPV infection, HPV 6+ 11 was the most common HPV genotype, accounting for 33.33 % (13/39) ; 5 women were found with HPV 11 + 18, accounting for 12. 82% ( 5/39 ) . Conclusion HPV 6, HPV 11, HPV 6 + 11, and HPV 11 + 18 are the main HPV genotypes of female anal condyloma acuminate. Gene amplification combined with gene chip technology are diagnostic methodsl suitable for HPV genotyping and detection, the sensitivity and specificity are good, which are suitable for molecular epidemiological study.
