福建地区健康女性血清HE4水平参考区间的探讨

摘　　要：目的探讨福建地区健康妇女血清人附睾上皮分泌蛋白4(HE4)水平的参考区间。方法采集福建地区693名健康体检女性及良性疾病女性患者血样,应用酶联免疫吸附试验方法检测血清HE4,分析年龄、绝经以及良性疾病对其影响,建立福建地区健康女性血清HE4水平参考区间。结果血清HE4水平随着年龄的增加而升高,两者呈显著正相关(r=0.272,P<0.01),与是否绝经亦呈显著相关关系(r=0.315,P<0.01);女性>50岁组血清HE4水平为(45.6±19.8)pmol/L,明显高于≤50岁组的(34.2±14.9)pmol/L;绝经后组血清HE4水平为(51.8±22.8)pmol/L,明显高于绝经前组的(34.3±14.2)pmol/L;健康体检组与良性妇科疾病组血清HE4水平无明显差异(t=1.49,P>0.05);福建地区健康女性血清HE4水平参考区间为≤50岁<58.6pmol/L、>50岁<78.2pmol/L;绝经前<58.1pmol/L、绝经后<89.1pmol/L。结论年龄以及是否绝经是影响女性血清HE4水平的重要生理因素,应根据不同年龄或是否绝经,分别建立血清HE4水平的参考区间。Objective To explore the reference ranges of human serum levels of human epididymis secretory protein 4 （HFA） in healthy women in Fujian area. Methods The blood specimens were abstracted from 693 healthy women after physical examination and fe- male patients with benign diseases in Fujian area, ELISA was used to measure serum HFA levels; the effects of age, menopause, and benign diseases on serum HE4 levels were analyzed, the reference ranges of human serum HFA levels in healthy women in Fujian area were established. Results Serum HFA levels increased with age, there was a positive correlation （r = 0. 272, P〈0. 01 ）, which was significantly correlated with menopause or not （ r = 0. 315, P〈0. 01 ） ; serum HE4 level among the women more than 50 years old was （45.6 ± 19. 8 ） pmol/L, which was significantly higher than that among the women aged 50 years old or less than 50 years old [ （34. 2±14. 9） pmol/L] ; serum HFA level among postmenopausal women was （51.8 ±22. 8 ） pmol/L, which was significantly higher than that among premenopansal women [ （34. 3 ±14. 2 ） pmol/L] ; there was no statistically significant difference in serum HFA level between the healthy women and the women with benign diseases （t= 1.49, P〉0.05） ; the reference ranges of human serum HFA levels in healthy women in Fujian area： 〈58.6 pmol/L （the women aged 50 years old or less than 50 years old） , 〈78.2 pmol/L （the women more than 50 years old） ; 〈58.1 pmol/L （premenopausal women）, 〈89. 1 pmol/L （postmenopausal women） . Conclusion Age and menopause or not are important physiological influencing factors of female serum HE4 levels, the reference ranges of human serum HE4 levels should be established respectively according to age and menopause or not of the women.
