摘  要:目的研究膀胱检查前不同准备状态及扫描体位对膀胱壁磁共振成像(MRI)图像质量的影响。方法 40例健康志愿者纳入研究,随机被分为4组,每组10例。4组检查者检查前分别行不同的准备或采用不同的扫描体位,对比T2WI矢状位、轴位、冠状位膀胱壁肌层两层信号的显示情况。结果两位图像分析者之间评分一致性良好。仰卧位检查时,检查者于检查前30 min适量饮水约150 ml较膀胱排空后即刻扫描图像得分高(P<0.05);检查前30 min均饮水约150 ml,仰卧位检查较俯卧位检查得分高(P<0.05);仰卧位检查时,检查前1 h排尿后禁水禁尿较检查前30 min饮水150 ml的图像得分高,但两者未见统计学差异;仰卧位或俯卧位检查,检查前30 min饮水150 ml较检查前1 h排尿后禁水禁尿检查图像得分无明显统计学差异。结论患者检查前1 h排尿后禁水禁尿行仰卧位MRI检查,有利于提高膀胱MR图像质量。Obijective To evaluate the effects of different subjects preparation and scanning position on bladder muscle wall detection when performed on a 3.0T MR scanner. methods Total 40 subjects ranged 22-40 years old were enrolled and randomly divided into four groups, subjects in each group had different preparations either for bladder filling or scanning position. Images obtained from each subject were interpreted based on the presentation of two different signal intensity layers of bladder muscle wall manifested on three vertical planes (sagittal, axial and coronal planes) of T2-weighted fat-suppressed images. Scores comparisons between each group were made. Resuit Image interpretation agreement between two readers was good. When scanning was performed on a supine position, image score obtained by taking moderate volume (150 ml) water 30 minutes before scanning were larger than those scanning image obtained immediately alter urinating (P〈0.05). Scores were also compared between two groups with examination performed on supine versus prone position when both taking moderate volume (150 ml) water 30 minutes before scanning, which were significantly larger in the former (P〈0.05). When subjects in both group were scanned in a supine position, although image scores were larger when subjects were examined 1 hour after urinating followed by a prohibition of water taking and urinating than those obtained through scanning performed 30 minutes alter moderate volume water taking, but there were no significant differences. No matter subjects either in supine or prone position, there were no significant differences between moderate water taking group and those prepared with urinating followed by water as well as urination prohibition 1hour before scanning. Conclusion Moderate bladder filling condition is necessary for bladder MR examination. Manifestation can be better when examination were performed in supine position rather than prone position.
