摘 要:目的拟通过动物实验探讨枸橼酸氢钾钠对三聚氰胺所致结石的抑制作用。方法将36只Wistar大鼠随机分为3组,每组12只。对照组每只给予2 ml/d蒸馏水灌胃;模型组每只给予1.0 g/(kg·d)三聚氰胺配成2 ml液体灌胃;干预组每只给予1.0 g/(kg·d)三聚氰胺及1.0 g/(kg·d)枸橼酸氢钾钠配成2 ml液体灌胃。给药时间为30 d。在第30天收集三组大鼠24 h尿液,测定尿p H值,采集血液行血肌酐、血尿素氮、血尿酸、胱抑素-C及β2-微球蛋白含量测定,30 d后将大鼠肾脏做病理切片观察比较。结果 (1)模型组24 h尿量较对照组减少,干预组24 h尿量均高于对照组和模型组。模型组比对照组尿p H值低,而干预组尿p H值均高于对照组及模型组,差异有统计学意义(P<0.05);(2)模型组肾脏体积变大,而体重减轻,模型组肾体比较对照组及干预组肾体比均增高;(3)与对照组和干预组比较,模型组血肌酐、血尿素氮、血尿酸、胱抑素-C及β2-微球蛋白均升高;(4)对照组病理切片未发现结晶颗粒,模型组可见大量结晶体,大部分集中在肾小管区,可见许多炎性细胞,肾小管扩张、上皮细胞受损;干预组未发现结晶体,仅见少数肾小管轻度扩张以及上皮细胞肿胀。结论 (1)三聚氰胺可以诱导大鼠出现结晶尿并损害肾功能;(2)枸橼酸氢钾钠可有效抑制Wistar雄性大鼠泌尿系统结石晶体形成,改善三聚氰胺对大鼠肾功能的损害。Obijective To explore the effects of potassium sodium hydrogen citrate on melamine stones by animal experiments. methods 36 Wistar rats were randomly divided into three groups, 12 rats in each group. Rats in control group were given 2 ml distilled water per day per rat, model group 1.0 g/kg melamine configured into 2 ml liquid per day per rat, prevention group 1.0 g/kg melamine and 1.0 g/kg potassium sodium hydrogen citrate configured into 2 ml liquid per day per rat. All the rats were given by gavage for 30 days. On the 30 th day, 24 h urine output and urine pH value were collected and measured in 24 h urine, serum creatinine, blood urea nitrogen, serum uric acid, cystatin-C and 132-micro-globulin content in blood were measured. Renal biopsy of rat were observed and compared after 30 days. Resuit (1)24-hour urine output in the model group compared with the control group decreased, 24-hour urine of prevention group were higher than control group and model group; (2)The body weight of model group rats was reduced, but kidney volume larger, kidney body ratio of model group increased compared with the control group and the prevention group rats; (3)serum creatinine (Scr), blood urea nitrogen (BUN), serum uric acid (UA), 132-microglobulin content and cystatin-C in model group were increased compared with the control group and the preventiongroup; (4)Biopsy of the control group showed no crystalline particles, biopsy of the model group showed a large number of crystalline particles, most of them concentrated in the tubular area,there were a large number of inflammatory cells, tubular dilatation, epithelial damage. No crystals, only some tubular mild expansion and swelling of the epithelial cells were found in prevention group. Conclusion (1)Melamine may induce crystallization of urine and impair renal function, the crystallization mainly focus on the location of the renal tubules; (2)Potassium sodium hydrogen citrate orally can effectively inhibit formation of crystals in Wist
