家庭无创通气治疗慢性阻塞性肺疾病稳定期合并慢性Ⅱ型呼吸衰竭患者疗效分析

摘　　要：目的:探讨慢性阻塞性肺疾病(COPD)稳定期并呼吸衰竭患者家庭无创正压通气的治疗作用。方法:研究纳入符合标准的COPD患者152例,随机分为治疗组76例和对照组76例,治疗组给予家庭无创正压通气及常规治疗,对照组仅给予常规治疗。随访1年,观察两组治疗前后的血气分析、肺功能、CAT问卷评分、COPD急性加重频率和平均住院次数的变化。结果:与对照组相比,治疗组患者1年后血气分析、CAT问卷评分、COPD急性加重频率和平均住院次数指标均有明显改善,肺功能下降速度减慢。结论:家庭无创辅助通气治疗能够改善稳定期COPD患者的血气分析、CAT问卷评分、COPD急性加重频率、平均住院次数,延缓肺功能下降速度。Objective： To investigate the effect of treatment Noninvasive positive pressure ventilation（NIPPV） at home in patients with chronic obstructive pulmonary disease（COPD）in the stable phase and respiratory failure.Methods：152 cases accord with standard of COPD patients were randomly divided into treatment group（n=76）and control group（n=76）,both groups were given routine treatment,the treatment group were given NIPPV on the basis of routine treatment.Then observed dyspnea,blood gas analysis and pulmonary function etc. after ventilator used in 1year.Results：In contrast with the control group,blood gas analysis、The CAT questionnaire score、Frequency of COPD exacerbations、The average In the hospital and Lung function decline slowed of the treatment group were evidently improved at the end of 1 year of therapy.Conclusion：The treatment of home NIPPV can improve the parameters of blood gas analysis、The CAT questionnaire score、Frequency of COPD exacerbations、 The average In the hospital and Lung function decline slowed as well as the daily symptoms in COPD patients in the stable phase.
