摘  要:目的评价转铁蛋白检测在验证大便少量或难辨白细胞中的价值。方法选择2013-2014年重庆医科大学附属第一医院住院患者190例,其中42例大便镜检仅见少量白细胞为实验组,148例大便镜检未见异常为对照组,分别检测其便隐血和粪便转铁蛋白。结果 42例实验组大便中,41例转铁蛋白阳性,1例转铁蛋白阴性,6例便隐血阳性,36例阴性;148例对照组大便中,5例转铁蛋白阳性,143例转铁蛋白阴性,3例便隐血阳性,145例阴性。转铁蛋白预测消化道炎症的阳性预测值为89.1%,敏感度为97.6%,特异度为96.6%。结论转铁蛋白检测阳性可验证大便镜检中存在的少量或难辨认白细胞,作为消化道炎症状态的筛查项目。Objective To investigate the value of transferrin detection in verifying a few or illegible white blood cells(WBC)in feces.Methods 190 inpatients in the First Affiliated Hospital of Chongqing Medical University were selected,among them 42 patients with a few WBC detected by fecal microscopic examination were taken as the experimental group and 148 patients without abnormality detected by the fecal microscopic examination were taken as the control group.The fecal occult blood and transferring were detected in the two groups.Results Among 42 cases in the experimental group,41 cases were transferrin-positive and 1case was transferrin-negative,6cases were fecal occult blood positive and 36 cases were fecal occult blood negative;among 148 cases in the control group,5cases were transferrin-positive and 143 cases were transferrin-negative,3cases were fecal occult blood positive and 145 cases were fecal occult blood negative.The positive predictive value,sensitively and specificity of fecal transferrin for predicting gastrointestinal inflammation was 89.1%,97.6%and 96.6%respectively.Conclusion The positive detection of fecal transferrin could verifying a few or illegible WBC existing in feces by fecal microscopic examination and can be used as a screening item for digestive tract inflammation.
