摘  要:目的探讨磁共振成像(MRI)选择性激励序列(PROSET)在坐骨神经成像中的信号特征,及其对坐骨神经病变的诊断价值。方法收集本院2008年10月至2012年5月收治的49例患者作为研究对象。所有患者均先行常规MRI检查,再采用PROSET序列进行腰骶部冠状面成像,得到坐骨神经影像。观察病变与坐骨神经的关系。结果 6例健康志愿者坐骨神经PROSET图像未见异常,形态显示清晰,信号对比良好,在诊断信息上对比STIR图像、T2图像有一定优势。外伤者PROSET图像显示良好的比例显著多于STIR图像、T2图像,差异有统计学意义(P<0.05)。其余患者PROSET图像可清楚地显示肿瘤形态及其与神经关系,STIR图像、T2图像显示不清晰。结论 PROSET序列可提升坐骨神经磁共振成像的清晰度,获得具有高对比、高空间分辨率特征的图像,在坐骨神经影像显示方面较常规序列更有利于临床诊断。Objective To investigate the morphological features and the diagnostic value of MRI principle of selective excitation technigue (PROSET) sequence in the sciatic nerve. Methods 49 patients admitted to our hospital from October 2008 to May 2012 were selected. All patients were firstly implemented conventional MRI examination, and than underwent lumbosacral portion coronal imaging by PROSET imaging technique in order to collect the sciatic nerve imaging. The relationship between lesion and sciatic nerve was observed. Results The PROSET images of sciatic nerve of 6 healthy volunteers showed normal and clear imaging, with good signal comparison, and the diagnostic messages were better than those of STIR images and T2 images. The percentage of good PROSET images of injury patients was significantly higher than that of STIR images and T2 images, with statistical difference (P〈0.05). The PROSET images of other patients clearly showed the relationship between tumor morphology and the nerve, but the STIR images and T2 images of these patients were not clear. Conclusions PROSET sequence can improve the clarity of the sciatic nerve magnetic resonance imaging, and gain imaging with high contrast and high spatial resolution, which is better than conventional sequence and helpful for clinical diagnosis.
