摘  要:目的探讨术中超声在颅脑肿瘤显微手术中定位、实时监测及引导切除中的应用价值及其优缺点。方法 45例颅脑肿瘤患者,术中超声确定肿瘤的位置、手术切除范围及切除术后是否有残余肿瘤。结果 45例脑肿瘤患者,术中超声均能清晰显示肿瘤的位置及性质,对病灶的显示率为100%;与术前MRI、CT对照,术中超声探查45例肿瘤部位、大小与其基本一致,8例开颅后与术前影像定位发生不同程度的漂移,术中超声重新准确定位。经手术最大程度切除肿瘤,均未损伤颅脑功能区。结论术中超声可准确确定肿瘤的部位及切除范围,提高手术安全性及治疗效果,是神经外科手术重要的辅助手段之一。Objective To evaluate the value of intraoperative ultrasonography in positioning,monitoring and guiding resection during microsurgery of brain tumors and to evaluate the advantages and disadvantages.Methods Fortyfive patients with brain tumor were enrolled.Intraoperative ultrasonography was performed to determine tumor location,range of excision and remnant tumors after resection.Results In 45 patients with brain tumor location,the location and character were clearly revealed by intraoperative ultrasonography.The display rate of lesions was 100%.In 45 patients,the tumor location and size revealed by intraoperative ultrasonography were basically consistent with preoperative MRI and CT.Tumor locations of 8 patients after craniotomy were different from preoperative image position,and tumors were repositioned by intraoperative ultrasonography.After resecting the tumor to the greatest extent,cerebral functional areas were not damaged.Conclusion Intraoperative ultrasonography can help surgeons identify the location and boundary of tumors and preserve the cerebral functional areas.It enhances the security and efficacy of the resection of brain tumors and becomes one of important assistant approaches in neurosurgery.
