房室结双径路1∶2房室传导体表心电图分析

摘　　要：目的 分析房室结双径路1∶2房室传导体表心电图特征.方法 回顾性分析山东省日照市人民医院1990年6月至2014年12月9例房窒结双径路1∶2传导患者体表心电图特征.结果 4例为窦性心律,1∶2房室传导;2例为窦性心律1∶1与1∶2传导交替发生;1例为窦性心律时而沿慢径路1∶1传导,时而1∶2传导,偶尔沿快径路以1∶1传导;1例为非阵发性房性心动过速,1∶2房室传导,偶尔沿快径路以1∶1传导,1∶0传导1次;1例为窦性心律,以稳定间期进行快径路和慢径路传导,表现为伪文氏现象.结论 体表心电图表现为房室结双径路1∶2传导少见,多为窦性心律,本组1例为非阵发性房性心动过速出现房室结双径路1∶2传导,偶发1∶0传导。Objective To analyze the characteristics of surface electrocardiogram （ECG） of dual atrioventricular （AV） nodal pathways with 1 ∶ 2 conduction.Method ECG data of 9 patients of dual AV nodal pathways with 1 ∶ 2 conduction were retrospectively analyzed.Results There were 4 cases with sinus rhythm showing 1 ∶ 2 AV conduction;2 cases with sinus rhythm showing alternative 1 ∶ 1 and 1 ∶ 2 AV conduction;1 case with sinus rhythm showing 1 ∶ 1 AV conduction along the slow pathway or 1 ∶ 2 AV conduction, occasionally 1 ∶ 1 AV conduction along the fast pathway;1 case with non-paroxysmal atrial tachycardia showing 1 ∶ 2 AV conduction, occasionally 1 ∶ 1 AV conduction along the fast pathway, 1 ∶ 0 AV conduction once;1 case with sinus rhythm showing AV couduction along the fast and slow pathway with a stable cycle, manifesting as pseudo Wenckebach＆#39;s phenomenon.Conclusion Dual AV nodal pathways with 1 ∶ 2 conduction is rare in surface ECG, mostly being sinus rhythm;this study reports 1 case with non-paroxysmal atrial tachycardia showing 1 ∶ 2, occasionally 1 ∶ 0 AV conduction.
