不同ICU实施呼吸机管道更换技术现状分析

摘　　要：目的调查国内多家ICU实施呼吸机管道更换技术的状况,为制定更好的呼吸机管道更换方法及流程提供依据。方法采用问卷调查法,根据ICU呼吸机管道更换技术自制调查问卷,对国内来自45家三级甲等医院ICU的60名护士进行问卷调查。问卷调查主要针对ICU类别、ICU实施呼吸机管道系统有关的内容。结果发放问卷60份,收回有效问卷57份。呼吸监护室(RICU)、急诊监护室(EICU)、外科监护室(SICU)和综合ICU护士在管道更换时机、管道更换方法、管道更换后操作方面比较差异均有统计学意义(P<0.05)。而在这四种ICU内,管道更换时使用人数和管道更换前处理比较差异均无统计学意义(P≥0.05)。在四种ICU内,大部分均采用双人更换,且更换管路前吸入纯氧。结论不同ICU在管道更换时机、管道更换方法、管道更换后操作方面间存在差异,无统一的操作。而不同ICU大部分均采用双人更换管道,且管道更换前吸入纯氧。Objective To investigate the status of ventilator tube replacement technology in different ICUs, and then provide basis for better ventilator tube replacement method. Methods Questionnaires on ICU venti-lator tube replacement technology were applied, and 60 nurses from ICUs of 45 domestic first-class hospitals were surveyed in this study. The qestionnaires focused mainly on ICU category and ventilator tube replacement technolo-gy. Results A total of 60 copies of questionnaires were sent and 57 valid questionnaires was taken back. Among nurses from respiratory intensive care unit （RICU）, emergency intensive care unit （EICU）, surgical intensive care unit （SICU） and integrated ICU, there were statistically significant differences in the timing of replacement, method of re-placement and operation after replacement （P〈0.05）. But there were no statistically significant difference in the num-ber of nurses who used the machine and operation before replacement. During most replacements in these ICUs, two nurses were needed and pure oxygen were breathed in before replacement. Conclusion There are differences in the timing of replacement, replacement method and operation after replacement between each ICU. And in most ICUs, two nurses are needed for the replacement and pure oxygen should be breathed in before replacement.
