摘 要:目的比较虚拟器官计算机辅助分析(VOCAL)软件对绝经后妇女子宫内膜容积测量与二维子宫内膜厚度测量在诊断子宫内膜良恶性病变的价值。方法收集绝经后妇女阴道出血或二维超声发现宫腔占位、内膜不均匀性增厚病例58例,正常对照组30例,行经阴道超声检查,二维式下测量内膜厚度,三维式下行内膜容积测量,所有研究对象均经宫腔镜检查或手术后病理证实。结果 1正常对照组与内膜良性病变组、内膜恶性病变组,在年龄、产次、孕次、绝经年限、有无肥胖、高血压、糖尿病等高危险因素,3组间差异均无统计学意义。2子宫内膜厚度、体积,3组间比较差异有统计学意义。3恶性病变组与良性病变组在彩色多普勒血流参数、血流分级上比较,差异有统计学意义;子宫内膜癌11例,III级血流显示率81.81%(9/11)。4对照病理诊断结果,三维超声诊断符合率高于二维超声(P<0.05)。结论三维超声对绝经后妇女子宫内膜良恶性疾病鉴别诊断准确性较二维超声提高,子宫内膜容积测量在子宫内膜良恶性病变的鉴别诊断中有较高的临床价值。Objective To compare the virtual organ computer- aided analysis software for postmenopausal endometrial volume measurement with 2D endometrial thickness measurement in the value of diagnosis for endometrial benign and malignant lesions.Methods Collecting 58 cases with postmenopausal vaginal bleeding or uterine placeholder and 30 healthy as normal control group. And intimal thickening of inhomogeneity found by 2D ultrasound and 3D via vaginal ultrasound examination,and 2D ultrasonic measured endometrial thickness,3D ultrasound measured endometrial volume,all of the objects had hysteroscopy examination or surgery pathology verification. Results The high risk factors of age,parity,gravidity,menopausal period,obesity,high blood pressure,diabetes had no statistical significance among normal control group,endometrial benign group and endometrial lesions group. The endometrial thickness and volume had statistical differences among those three groups. The malignant lesions group and begin lesions group had statistically significances on color Doppler parameters of blood and TIBI endometrial cancers were in 11 cases,III level rate of blood flow was 81. 81%( 9 /11). Comparing the pathological diagnosis,3D ultrasound diagnosis coincidence rate was higher than 2D ultrasound( P〈0. 05). Conclusion 3D ultrasound used for postmenopausal endometrial benign and malignant disease diagnosis has higher accuracy compared with 2D ultrasound. Endometrial volume measurement has higher clinical value in the differential diagnosis of benign and malignant lesions of the film inside the uterus.
