胰头血供三种研究方法的比较

摘　　要：目的对显示胰头血供的3种研究方法进行比较,为胰头癌的治疗提供解剖学参考依据。方法新鲜的成人整尸标本,分别行红色乳胶灌注;聚乙烯醇—氧化铋灌注并行X线显影和CT扫描,所得CT数据以Dicom格式输入Minics15.0用于胰头血供的三维重建。游离脾胰联合标本行过氯乙烯灌注,用于管道铸型。结果3种方法都能清晰显示胰头内外血供及其分支,但各有其特点:乳胶灌注能清晰显示胰头外部的血供情况;过氯乙烯铸型标本可显示胰头内部的血供;聚乙烯醇—氧化铋放射显影能清晰显示胰头内外血供的分布、吻合情况。结论乳胶灌注法、管道铸型、动脉造影都能清晰显示胰头内外血供的特点;3种方法分别适用于显示胰头外部、内部和整体的血供分布吻合情况。Objective To compare the three kinds of methods on the blood supply in head of pancreas,and provide the anatomical basis for the treatment of the carcinoma of head of pancreas. Methods Fresh adult specimens with arteries perfused with red lactoprene,polyvinyl alcohol（ PVA） and the bismuth oxide,respectively,the latter were subjected to spiral CT scanning and X-ray imaging,and CT data were inputed to Minics 15. 0 and 3D reconstruction was conducted to obtain 3D vessels in head of pancreas. Free spleen pancreas joint specimens with arteries perfused with perchloroethylene to form cast specimen. Results The arteries and those branches in head of pancreas can be showed clearly in these three kinds of methods,but each shows its characteristics. As follows,the external vessels can be clearly displayed on the latex perfusion specimens; the internal vessels can be showed on the cast specimens; and blood supply distribution and anastomosis in head of pancreas can be clearly displayed on radiography. Conclusion The blood supply in head of pancreas can be clearly showed through latex perfusion,vessels casting,and angiography,three kinds of methods can display the distribution anastomosis of the external,the internal,and the whole blood supply in head of pancreas.
