摘 要:目的以居民电子健康档案系统为研究平台,探讨社区高血压远程监测管理效果。方法 2014年3月从张家港市居民电子健康档案系统中分层随机抽取两组原发性高血压患者作为对照人群和干预人群。对对照组采用社区高血压常规管理,干预组开展"高血压远程监测管理"干预。一年后随访比较两组实施前、后高血压知识知晓情况、行为改变(遵医服药、盐摄入控制、适量运动以及饮酒控制)及高血压分级与控制情况。结果干预实施前,631例干预患者与650例对照患者在高血压知识知晓情况、相关行为及高血压分级与控制方面差异均无统计学意义(P>0.05)。干预实施一年后,高血压知识问卷得分指标干预组高于对照组,差异有统计学意义(P<0.01);遵医服药率、盐摄入控制率、适量运动率以及饮酒控制率指标干预组均高于对照组,差异有统计学意义(P<0.05);在高血压分级分布上,干预组中、重度高血压比例低于对照组,差异有统计学意义(P=0.03);在高血压控制率上,干预组控制率高于对照组,差异有统计学意义(P<0.01)。结论基于居民电子健康档案平台的高血压远程监测管理是开展社区高血压综合防治的有效方法。Objective To investigate the effect of remote monitoring management of hypertension in community based on community electronic health records system. Methods Two groups of essential hypertension patients were randomly selected as control group and intervention group in 2014 March from the electronic health records system of Zhang Jiagang. Routine community management of hypertension was used in the control group, while the "hypertension remote monitoring and management" was carried out in the intervention group. One year later, hypertension knowledge, behavior change and hypertension classification distribution were compared between two groups before and after the intervention. Results Before the intervention, there was no statistical significance difference(P〈0.05) between 631 cases of intervention patients and 650 cases of control in patients with hypertension knowledge, the related behavior, hypertension classification and hypertension control rate. One year later after intervention, the average score in hypertension knowledge questionnaire of the intervention group were higher than the control group, and the difference was statistically significant(P〈0.01). The rates of intervention group were higher than those in the control group on the drug compliance,control of salt intake, moderate exercise and control of drinking respectively, and the difference was statistically significant(P〈0.05).The proportion of moderate hypertension and severe hypertension of the intervention group were lower than the control group, and the difference was statistically significant(P=0.03). Finally, the hypertension control rate of intervention group was higher than control group after the intervention, and the difference was statistically significant(P〈0.01). Conclusion Hypertension remote monitoring and management based on community electronic health records system is the effective approach to integrated control community hypertension.
