聚丙烯酰胺水凝胶注射后远处移位的临床特点与治疗

摘　　要：目的：分析12例聚丙烯酰胺水凝胶（奥美定,PAMHG）注射后并发注射物远处移位的临床表现,探讨其处理方法。方法：12例患者中乳房注射10例,颞部填充2例;出现注射物移位的部位：腹壁、大腿及会阴部9例,面颊部2例,乳沟区1例,均采用手术切开,显露注射腔,大量盐水冲洗;留置引流、持续灌洗。结果：所有病例的术前自觉症状消失,1例经MRI显示注射物残留而进行二次手术,1～5年随访无异常。结论：聚丙烯酰胺水凝胶注射后并发症多在原发部位发生,注射物远处移位也不少见,采用开放手术处理是切实有效的治疗方法。Objective To investigate the clinical manifestations of the shift complications after polyacrylamide hydrogel injection,and the treatment. Methods 12 patients were included in this study,10 of breast augmentation,and 2 of temporal augmentation.The polyacrylamide hydrogel had moved to abdominen,thigh and perinaeum in 9 patients,2 to cheek,and 1 to cleavage area.All patients were treated by surgical operations to remove the injected PAMHG,incision drainage and continous lavage. Results Almost all the self-symptoms had disappeared after the operation.One patient received secondary surgery.And there was no difference in the next 1-5 years following. Conculsion The complications of PAMHG injection mainly occurred in situ, also related to remote displacement in the clinical.Surgery is a better treatment for the complications of PAMHG injection.
