摘 要:目的了解医院重症监护病房(ICU)医院感染的分布及其影响因素,为采取有效干预措施提供参考。方法采用前瞻性监测方法,对某医院ICU住院患者医院感染情况进行监测,并对集束干预措施的效果进行观察。结果共监测该医院ICU入住危重症患者948例,监测到医院感染患者241例,医院感染发生率25.42%。该ICU使用呼吸机的患者呼吸机相关性肺炎(VAP)发生率平均为11.35%。通过集束干预措施,各年度医院感染率出现逐年下降趋势。该医院ICU感染患者送检标本中分离出的多重耐药菌居前5位的是鲍曼不动杆菌、铜绿假单胞菌、肺炎克雷伯菌、大肠埃希菌和耐甲氧西林金黄色葡萄球菌(MRSA)。结论该医院ICU住院患者医院感染发生率较高,经采取集束干预措施使得感染率呈现逐年下降趋势。Objective To neference for explore the distribution and the influencing factors of nosocomial infection in intensive care unite( ICU),so as to providie effective interventional measures. Methods A prospective method was used to monitor the nosocomial infection in ICU of the hospital and observe the effect of cluster intervention. Results Of 948 patients in ICU of the hospital,241 suffered from nosocomial infection with the incidence rate of 25. 42%. The incidence rate of ventilator associated pneumonia( VAP) in the patients in ICU was 11. 35%. The incidence rate of nosocomial infection decreased year by year by means of cluster intervention measures. The multiple resistant bacteria isolated from the specimens in ICU patients with nosocomial infection were mainly Acinetobacter baumannii,Pseudomonas Aeruginosa,Klebsiella pneumonia,Escherichia co1 i and Methicillin-resistant Stphylococcus aureus( MRSA). Conclusion The incidence rate of nosocomial infection in ICU patients is high. The incidence rate of nosocomial infection in ICU patients decreased year by year by means of cluster intervention measures.
