摘  要:目的探讨新生儿苍白球磁共振成像T1加权(T1WI)信号对称性升高的原因。方法回顾性分析427例新生儿磁共振图像,对苍白球T1WI信号进行定性和定量分析。苍白球T1WI信号升高与各种临床病因之间的关系采用Logistic回归分析,同时分析苍白球/大脑皮质T1WI信号强度值与血清总胆红素(TSB)水平、日龄之间的关系。结果 427例中249例(58.3%)出现苍白球不同程度的T1WI信号升高;Logistic回归分析显示高胆红素血症的回归系数B=1.777,OR值=5.915(P<0.01);290例高胆红素血症中209例(72.1%)出现苍白球T1WI信号升高,137例无高胆红素血症者中40例(29.2%)信号升高。进一步分析显示随着TSB水平的升高,苍白球/大脑皮质信号强度值有升高趋势,差异有统计学意义(χ2=84.734,关联性系数r=0.407,P<0.01)。相关性分析表明苍白球/大脑皮质信号强度值与日龄呈负相关(r=-0.441,P<0.01)。结论新生儿苍白球T1WI信号升高主要由TSB水平升高引起,可能还与其他因素有关。Objective To investigate the causes of symmetric hyperintensity of the globus pallidus on T1-weighted (T1WI) magnetic resonance images in newborns. Methods The brain magnetic resonance images of 427 neonates admitted in our hospital from January 2013 to June 2014 were reviewed, and the T1WI signal patterns in the globus pallidus were qualitatively and quantitatively analyzed. Logistic regression analysis was applied to investigate contributions of the clinical factors to the hyperintensity of globus pallidus on T1WI. The signal intensity ratio of globus pallidus to cerebral cortex was compared with the total serum bilirubin (TSB) levels and the postnatal age at examination. Results T1WI hyperintensity of the globus pallidus was found in 249 of 427 cases (58.3%). Logistic regression analysis showed that regression coefficient of hyperbilirubinemia=1.777, OR=5.915 (P〈0.01). T1WI hyperintensity of the globus pallidus was found in 209 of 290 cases with hyperbilirubinemia (72.1%) and 40 of 137 cases without hyperbilirubinemia (29.2%). Further analysis showed that the signal intensity ratio of the globus pallidus to the cerebral cortex was increased with the increase of TSB level (Chi square=84.734, P〈0.01). Correlation analysis showed that the signal intensity ratio of the globus pallidus to the cerebral cortex was negatively correlated with postnatal age at MRI examination (r=-0.441, P〈0.01). Conclusion The T1WI hyperintensity of globus pallidus in newborns is mainly caused by the elevated TSB levels, and may also be related to other factors.
