来曲唑联合二甲双胍治疗多囊卵巢综合征不孕症的疗效观察

摘　　要：目的：探讨来曲唑联合二甲双胍治疗多囊卵巢综合征（PCOS）不孕症的疗效。方法：选择2012年6月-2013年6月期间我院收治的PCOS不孕症女性150例,随机分为研究组75例与对照组75例。对照组单纯采取来曲唑促排卵治疗,研究组采取来曲唑联合二甲双胍治疗。观察对比两组治疗前后糖代谢情况、临床疗效及排卵结局。结果：治疗前两组空腹胰岛素、空腹血糖、胰岛素抵抗（IR）比较无统计学差异（P〉0.05）;治疗后研究组空腹胰岛素、空腹血糖、IR均显著降低,且研究组著低于对照组（P〈0.05）。研究组排卵数、优势卵泡数显著高于对照组（P〈0.05）,卵泡生长时间显著低于对照组（P〈0.05）。研究组妊娠率为60.00%,显著高于对照组的33.33%（P〈0.05）。结论：来曲唑联合二甲双胍治疗PCOS不孕症疗效确切,可以有效降低雄激素水平,增加优势卵泡数量及排卵数量,保障了患者的生育功能及妊娠质量。Objective： To explore the effect of letrozole combined with metformin in the treatment of infertility with polycystic ovary syndrome（PCOS）. Methods： Selected 150 infertile women with PCOS who were treated in our hospital from June 2012 to June2013, they were randomly divided into study group and control group with 75 patients in each group. The control group was only treated with letrozole ovulation therapy, while the study group was treated with letrozole combined with metformin therapy. Observed the glycometabolism indexes, curative effect and ovulation outcome in two groups before and after treatment. Results： Fasting blood glucose,fasting insulin, insulin resistance（insulin resistance, IR） in two groups before treatment had no significant difference（P〈0.05）; Above indexes in study group after treatment were significantly decreased than before treatment, which were significantly lower than the control group after treatment（P〈0.05）. Ovulation amount, dominant follicle amount of study groups was significantly more than control group（P〈0.05）, and the follicular growth time was significantly shorter than control group（P〈0.05）. The pregnancy rate in study group was60.00%, which was significantly higher than 33.33% in control group（P〈0.05）. Conclusion： Letrozole combined with metformin is not only has a exact curative effect in the treatment of infertility with PCOS, but also can effectively reduce the androgen level,increase the ovulation amount and dominant follicle amount, which can improve the life and pregnancy quality of the infertility.
