甘肃省天水市MSM中HIV感染因素的配对研究

摘　　要：目的了解天水市男男性行为人群(MSM)对艾滋病的知识、态度和行为,分析其感染艾滋病病毒(HIV)的危险因素。方法对23名感染HIV的MSM,按照调查时间1∶4的比例进行配对,用SPSS统计软件采用Cox回归模型进行病例对照分析。结果本次共调查MSM 800例,HIV抗体确认阳性23例,匹配HIV阴性的MSM92例。单因素分析发现,两组在艾滋病知识的知晓率、近6个月与同性肛交的比例、与同性发生肛交性行为时坚持使用安全的比例、曾有同性商业性行为的比例的差异均有统计学意义(P<0.05);Cox回归分析发现,近6个月与同性发生肛交性行为者,感染HIV的比值比(OR)值为10.53;曾有同性商业性行为者感染HIV的OR值为2.95。结论近6个月与同性发生肛交性行为和曾有同性商业性行为是MSM感染HIV的危险因素。Objective To understand Men who have sex with men（MSM）AIDS knowledge,attitude and behavior,and to analyze the risk factors for HIV infection in Tianshui City.Methods A total of 23 HIV-infected MSM in accordance with the ratio of 1∶4paired were studied.SPSS statistical software was applied to conduct case-control analysis with Cox regression models.Results Among 800 cases of MSM,HIV antibody was confirmed positive in 23 cases,HIV-negative MSM 92 cases were matched according to the survey time.Unvaried analysis found statistically significant differences between the two groups（P 〈0.05）in terms of the percentages of knowledge of AIDS awareness,homosexual anal sex in last 6months,insisting on using of condom during homosexual anal sex,and commercial homosexual anal sex.Cox regression analysis found that those who had homosexual anal sex in last 6months,were infected with HIV with a ORof 10.53,and those who had commercial homosexual anal sex were infected with HIV with a ORof 2.95.Conclusion Homosexual anal sex in last 6months and commercial homosexual anal sex were risk factors for HIV infection in MSM.
