Analyze on the prevalence of HIV and syphilis among people receiving voluntary counsel and test for HIV in Yandu District of Yancheng City from 2011 to 2014

摘　　要：目的分析盐城市盐都区艾滋病自愿咨询检测(VCT)求询者人群的相关情况,以及艾滋病病毒(HIV)和梅毒检测结果,为掌握当地艾滋病流行因素,采取针对性预防控制措施提供科学依据。方法收集盐都区2011-2014年各VCT点求询者的个案信息,以及通过VCT点上报的HIV感染者和病人的个案信息,进行流行病学分析。结果 4年共有4635人接受了咨询和检测,共检出HIV抗体阳性21例(0.45%),梅毒抗体阳性149例(3.21%)。配偶/固定性伴阳性、男男性行为史、非商业非固定异性行为史和商业异性行为史人群的HIV抗体阳性率,分别为8.57%(3/35)、3.52%(9/256)、0.89%(4/448)和0.15%(5/3370)。梅毒抗体阳性求询者的HIV感染率(3.36%,5/149)高于梅毒抗体阴性者(0.36%,16/4486)的感染率(χ2=22.49,P=0.000)。经VCT点检出的HIV抗体阳性者首次CD+4T淋巴细胞计数>200个/μL的占80.95%(17/21)。结论盐都区VCT求询者中,性接触为主要的高危行为,需重点加强预防控制性途径传播艾滋病的措施。同时,VCT有利于早期发现HIV感染者/艾滋病病人,应进一步加强和规范咨询检测服务。Objective To analyze the general information and the HIV and syphilis test results of people who received human immunodeficiency virus（HIV）voluntary counseling and testing（VCT）in Yandu District of Yancheng City,and to make scientific suggestions for improvement of HIV/sexually transmitted diseases（STD）prevention and control.Methods Information on individual clients was collected from different VCT sites and epidemiological analysis was conducted among HIV/AIDS cases reported by VCT sites in Yandu District from 2011 to 2014.Results The VCT clinic provided VCT services for 4635 cases in total.The infection rate of HIV and syphilis was 0.45%（21/4635）and 3.21%（149/4635）respectively.The rates of HIV positive among HIV positive spouse/fixed sexual partner,men who have sex with men homosexuals,non-commercial and extramarital heterosexual and commercial heterosexual were 8.57%（3/35）,3.52%（9/256）,0.89%（4/448）and 0.15%（5/3370）.The rates of HIV positive in syphilis positive persons were significantly higher than in syphilis negative persons（χ2= 22.49,P=0.000）.The rate of the counts of CD4 more than 200/μL among the HIV/AIDS cases detected at VCT sites was 80.95%（17/21）.Conclusions Sexual contact history was the main HIV/AIDS related high-risk behaviors among VCT clients in Yandu district.We should strengthen the measures to prevention and control the spread of HIV/AIDS from sexual transmission.VCT clinics can find HIV positive patients in time.
