支撑喉镜下低温等离子射频治疗会厌囊肿疗效分析

摘　　要：目的探讨支撑喉镜下等离子射频消融治疗会厌囊肿的临床疗效。方法将2006年6月至2013年6月东莞市莞城医院收治的64例拟行手术治疗的会厌囊肿患者,按随机数字表法分为两组,每组32例,观察组采用支撑喉镜下低温等离子射频消融治疗,对照组采用传统手术切除,比较两组手术时间、术中出血量、术后出血量及术后疼痛时间,并比较两组手术前后会厌囊肿评分及术后复发率。结果两组均成功完成手术,术后手术区域黏膜光滑、平坦,观察组手术时间、术中出血量、术后出血量及术后疼痛时间均少于对照组[(9.4±2.8)min比(12.6±3.7)min,(2.4±1.1)m L比(11.2±3.9)m L,(0.2±0.1)m L比(12.8±0.8)m L,(1.3±0.7)min比(4.2±1.6)min,P<0.01];会厌囊肿评分两组治疗前差异无统计学意义(P>0.05),治疗后两组均较治疗前下降(P<0.01),观察组低于对照组[(0.7±0.3)分比(1.6±0.8)分,P<0.01];术后12个月时观察组无复发病例,对照组复发3例(9.4%),两组复发率比较差异无统计学意义(P>0.05)。结论支撑喉镜下低温等离子射频消融术可精确切割病变组织,手术时间短,出血量少,周围组织损伤小,近期复发率低,可作为会厌囊肿优选的手术方式。Objective To investigate the clinical effect of low-temperature plasma radiofrequency abla- tion on epiglottic cyst under laryngoscope. Methods A total of 64 patients with epiglottic cyst undergoin~ operation treatment in Guancheng Hospital from Jul. 2006 to Jul. 2013 were divided into two groups by ran- dom number table method, 32 cases in each group, the observation group was treated with low-temperature plasma radiofrequency ablation on epiglottic cyst under laryngoscope, the control group was treated with tradi- tional operation resection, and the operation time, intraoperative bleeding volume, postoperative bleeding anc postoperative pain, and epiglottic cysts score and postoperative recurrence rate before and after the operatior were compared. Results All the patients of the two groups successfully completed the operation,local muco- sa was smooth, flat after operation, and the operation time,intraoperative bleeding volume, postoperative pair time and postoperative bleeding of the observation group were less than the control group [ （9.4 ± 2.8 ） mir vs （12.6 ±3.7） min,（2.4 ± 1.1） mL vs （11.2 ±3.9） mL, （0.2 ±0.1） mE vs （12.8 ±0.8） mL, （ 1.3 ±0.7） rain vs （4.2 -± 1.6） min,P 〈0.01 ] ;there was no statistical significant difference in epiglotti cysts score between the two groups before treatment （P 〉 0.05 ）, and both groups decreased after treatmen （ P 〈 0.01 ）, the observation group was lower than the control group [ 0.7 ± 0.3 ） scores vs （ 1.6 ± 0.8 ） scores,P 〈 0. 01 ] ; there was no case of recurrence in the observation group at 12 months after operation anc there were 3 cases （9.4%）in the control group, which was higher than that of the observation group, but the difference was not statistically significant （ P 〉 0. 05 ）. Conclusion Low-temperature plasma radiofrequenc ablation on epiglottic cyst under laryngoscope can cut lesions accurately, which has the advantages of shor operation time, less bleeding volume, little injury of the
