摘 要:目的:观察脓毒症小鼠外周血单核细胞负性共刺激分子PD-L1及正性共刺激分子CD86的表达趋势,探讨脓毒症小鼠的免疫状态。方法60只C57小鼠随机分为6 h、24 h、48 h脓毒症模型组及假手术组,每组10只小鼠。采用盲肠结扎穿孔法造模,造模成功后分别于6 h、24 h、48 h取外周血,用流式细胞仪检测各组单核细胞PD-L1及CD86的表达,同时对小肠进行HE染色观察形态学变化。结果在100×光镜下观察24 h及48 h模型组小鼠小肠上皮黏膜绒毛破坏严重,萎缩明显,假手术组和6h模型组小肠黏膜绒毛正常。模型组6h、24h、48h单核细胞PD-L1及CD86的表达均较假手术组升高(P均<0.05),且PD-L1于24 h、48 h升高更明显(P<0.01)。模型组中,PD-L1及CD86均在6 h开始升高,24 h达峰值,48 h开始下降。结论脓毒症小鼠外周血PD-L1及CD86表达均升高,且PD-L1升高更明显,提示脓毒症早期机体特异性免疫主要呈现负性调节。Objective To observe the expression trend of negative costimulatory molecule programmed death ligand 1 ( PD -L1 ) and positive costimulatory molecule CD 86 in peripheral blood mononuclear cells of septic mice , to investigate the immune status of sepsis in mice .Methods Sixty C57 mice were randomly divided into 6 h, 24 h, 48 h sepsis model group and the sham group , 10 mice in each group .Sepsis model was reproduced by cecal ligation and puncture .6 h, 24 h, 48 h after successful modeling peripheral blood mononuclear cells were taken for detection of PD -L1 and CD86 by flow cytometry, while HE staining was used to observe the morphological changes of intestine .Results Under the 100 × light microscope , the 24 h and 48 h sepsis group showed serious intestinal epithelial mucosal villous damage , but the sham group and 6 h sepsis group showed normal small intestinal villi . CD86 and PD-L1 expression in sepsis model group in 6 h, 24 h, 48 h was all increased compared with that in sham group (P〈0.05).And elevated levels of PD-L1 in 24 h, 48 h is more significant (P〈0.01).CD86 and PD-L1 were increased at the beginning of 6 h, 24 h reached the peak, and 48 h began to decline .Conclusion Peripheral blood mononuclear cells in sepsis mice showed increased expression of CD86 and PD -L1, and the rise of PD -L1 level was even more significant , which suggests that negative regulation of specific immunity is dominant during the early stage of sepsis .
