择期手术病人输血前不规则抗体筛查结果及应用价值

摘　　要：目的探讨择期手术病人输血前不规则抗体筛查的临床意义。方法对香河县人民医院2011年1月—2014年7月13 416例择期手术病人进行输血前不规则抗体筛查,抗体筛查阳性者用特异性鉴定谱细胞做进一步鉴定。以2008年1月—2010年12月未进行输血前不规则抗体筛查的12 208例择期手术病人作为对照组,比较两组的输血反应发生率。结果筛查组13 416例病人共检出不规则抗体116例,检出率为0.865%（116/13 416）,其中检出特异性抗体101例,检出率为0.753%;非特异性抗体15例,检出率为0.112%。不规则抗体的检出率与性别、输血次数及妊娠史有关（χ2=10.28~69.21,P〈0.05）。筛查组输血反应发生率明显低于对照组,差异有极显著意义（χ2=26.37,P〈0.01）。结论择期手术病人有一定的不规则抗体检出率,输血前筛查有利于提高输血的安全性。Objective To explore the clinical significance of irregular antibody screening（IAS）before blood transfusion in patients scheduled for surgery. Methods Between January 2011 and July 2014,an IAS was conducted in 13 416 patientshospitalized in Xianghe County People’s Hospital-scheduled for surgery before blood transfusion,those with positive results were further determined using specific identification of spectral cells.A control group was consisted of 12 208patients-hospitalized between January 2008 and December 2010-who did not receive IAS before blood transfusion for surgery.The transfusion reaction rate between the two groups was compared. Results Of the 13 416 cases in the IAS group,irregular antibodies were detected in116 cases,the detection rate was 0.865%（116/13 416）,of which,specific antibodies being 101（0.753%）,and non-specific antibodies 15（0.112）%.The detection rates of irregular antibodies were associated with sex,the number of blood transfusion and a history of pregnancy（χ2=10.28-69.21,P〈0.05）.Blood transfusion reaction rate in the screening group was lower than that in the control group（χ2=26.37,P〈0.05）. Conclusion There is a certain detection rate of irregular antibodies in patients scheduled for selective operation,a screen of irregular antibodies before blood transfusion is conducive to increasing the safety of blood transfusion.
