摘 要:目的探讨转化生长因子-β1(Transforming growth factor-β1,TGF-β1)、结缔组织生长因子(Connective tissue growth factor,CTGF)与胆道闭锁(Biliary atresia,BA)患儿肝硬化程度的相关性。方法运用酶联吸附试验(ELISA法)检测胆道闭锁组和对照组患儿血清中TGF-β1及CTGF的表达。运用免疫组化法检测胆道闭锁组和对照组患儿肝脏组织中TGF-β1及CTGF的表达。结果胆道闭锁组患儿血清中TGF-β1、CTGF的含量明显高于对照组(P<0.05)。胆道闭锁组患儿术前血清中TGF-β1、CTGF的表达水平高于术后的表达水平(P<0.05)。随着肝纤维化的逐渐加重,胆道闭锁组患儿术前血清中TGF-β1、CTGF的表达水平逐渐升高,术前胆道闭锁患儿血清中TGF-β1、CTGF水平与肝纤维化呈正相关(r=0.712,P=0.021;r=0.581,P=0.039)。胆道闭锁组患儿肝脏组织中TGF-β1、CTGF的表达高于对照组,差异有统计学意义(P<0.05)。随着肝纤维化的逐渐加重,胆道闭锁组患儿肝脏组织中TGF-β1、CTGF的表达水平逐渐升高,且TGF-β1、CTGF表达水平与肝纤维化呈正相关(r=0.624,P=0.026;r=0.512,P=0.041)。结论 TGF-β1、CTGF在胆道闭锁患儿血清中有较高表达,TGF-β1、CTGF在胆道闭锁患儿肝脏组织中的也有较强的阳性表达。TGF-β1、CTGF密切参与胆道闭锁患儿肝纤维化的形成,TGF-β1、CTGF可以作为胆道闭锁患儿肝纤维化的潜在预判定指标。Objective To investigate the correlation of the transforming growth factorβ1(TGF-β1),connective tissue growth factor(CTGF)and degree of biliary atresia(BA)in children patients.Methods Using enzyme-linked assay(ELISA method)to detect the expression of TGF-β1and CTGF in serum of biliary atresia and control groups and immunohistochemistry to detect the expression of TGF-β1and CTGF in liver tissue of the groups.Results The expression level of TGF-β1and CTGF in biliary atresia group was significantly higher than control group(P 〈0.05).The expression level of TGF-β1and CTGF in serum of before surgery was higher than that of post-operative in the biliary atresia group(P 〈0.05).With the gradually increased hepatic fibrosis,the expression level of serum TGF-β1and CTGF were gradually higher in biliary atresia group before surgery,which was showing positive correlation(r=0.712,P =0.021 for TGF-β1;r=0.581,P =0.039 for CTGF vs hepatic fibrosis).The expression of TGF-β1and CTGF of children with biliary atresia in liver tissue were higher than control group that the difference was statistically significant(P 〈0.05).With gradually increased of hepatic fibrosis,TGF-β1and CTGF expression levels in liver in children with biliary atresia were gradually increased,which showed positive correlation(r=0.624,P =0.026 for TGF-β1;r=0.512,P =0.041 for CTGF vs hepatic fibrosis).Conclusion There were higher expressions of TGF-β1and CTGF in serum and liver tissue of children with biliary atresia.TGF-β1and CTGF were closely involved in the formation of liver fibrosis in children with biliary atresia,which could be potential pre-judgment indicators of liver fibrosis for biliary atresia children.
