摘　　要：目的探讨平均血小板体积(mean platelet volume,MPV)在诊断中青年急性心肌梗死及预测冠状动脉病变严重程度中的作用。方法选择2013年8月-2013年12月在新疆医科大学第一附属医院住院的中青年急性心肌梗死患者(年龄<60岁)131例为病例组,根据冠状动脉造影结果对病例组进行冠状动脉狭窄程度积分(Gensini积分)评价,其中高于Gensini积分中位数(P50=40分)有67例,低于P50(<40分)有64例。选择152例同时期本院健康体检者为对照组。比较病例组与对照组、病例组中Gensini积分高于P50的患者与Gensini积分低于P50患者的MPV,多元线性回归分析MPV与Gensini积分的相关性。结果病例组MPV水平为(10.27±1.27)fL,高于对照组的(9.60±1.50)fL(P<0.05)。病例组中Gensini积分高于P50患者的MPV水平为(10.51±1.32)fL,高于Gensini积分低于P50患者的(10.02±1.17)fL,(P<0.05)。多元线性回归显示,MPV与Gensini具有相关性(P<0.05)。结论 MPV在诊断中青年急性心肌梗死及预测冠状动脉病变严重程度方面具有一定的价值。Objective To investigate the relationship between mean platelet volume（MPV）and occurrence of acute myocardial infarction（AMI）,severity of coronary lesions in middle-young patients.Methods A total of 131 Young and middle-aged AMI inpatients（60year-old）of our hospital between August 2013 and December 2013 were recruited.The case group were further divided into two groups according to median of the modified Gensini score（P50=40）preoperatively.The two groups were above 50 percentile group（≥P50）and under the 50 percentile group（〈P50）.The control groups were 152 subjects of healthy volunteers.The level of MPV was compared between the case and control.Results MPV was significantly higher in case group than in control group（10.27±1.27）vs（9.6±1.5）fL（P 〈0.05）.MPV level in P50group was higher than in P50group（10.51±1.32）vs（10.02±1.17）fL（P 〈0.05）.According to Multiple Linear Regression,there was strong correlation between the level of MPV and severity of coronary lesions in middle-young AMI patients（P 〈0.05）.Conclusion MPV is not only significantly associated with development of AMI in middle-young patients,but also can be a reliable predictive index for severity of coronary lesions.
