金匮肾气汤联合苓桂术甘汤治疗慢性阻塞性肺疾病(脾肾阳虚)的临床疗效分析

摘　　要：目的:比较金匮肾气汤联合苓桂术甘汤与单纯金匮肾气汤对慢性阻塞性肺疾病(脾肾阳虚)的临床效果,为临床治疗提供参考。方法:收集来我科就诊的临床诊断为慢性阻塞性肺疾病(脾肾阳虚)的患者72例,采用随机数字表法将其分为两组,分别为金匮肾气汤联合苓桂术甘汤治疗组与单纯金匮肾气汤治疗组,每组36例,对两组患者临床主要症状得分及中医症候总积分、临床疗效及生活质量进行比较。结果治疗前两组患者在咳嗽、咳痰、胸闷、气促得分及总积分之间的差异无统计学意义(P>0.05),治疗后观察组患者在咳嗽、咳痰、气促得分及总积分均低于对照组,差异具有统计学意义(P<0.05);观察组的总有效率为91.7%,对照组的总有效率为72.2%,两组患者在临床总有效率之间的差异具有统计学意义(χ2=4.600,P=0.032);观察组与对照组患者在生理、心理、日常活动、社交娱乐、运动、工作学习及家庭方面的差异有统计学意义(P<0.05),在性生活及治疗方面的差异无统计学意义(P>0.05)。结论金匮肾气汤联合苓桂术甘汤治疗慢性阻塞性肺疾病(脾肾阳虚)的临床效果显著,推荐在临床上应用。Objective： To compare the clinical curative effect of Jingui Shenqi Decoction combined with Linggui Zhugan Decoction with Jingui Shenqi Decoction in the treatment of chronic obstructive pulmonary disease（ spleen- kidney Yang deficiency） so as to provide the reference for clinical treatment. Method： 72 cases of patients from our department who were diagnosed as chronic obstructive pulmonary disease（ spleen- kidney Yang deficiency）,according to the random number table method,were divided into two groups. One group were treated with Jingui Shenqi Decoction combined with Linggui Zhugan decoction while the other were treated with simple Jingui Shenqi Decoction,36 cases in each group. Compare clinical main symptoms score,traditional Chinese medicine symptoms integral,clinical curative effect and quality of life of two groups. Results： Before the treatment,the score and total integral of two groups had no statistically significant difference in cough,sputum,chest tightness and shortness of breath（ P〉 0. 05）. After treating,the scores and total integrals of coughing,coughing up phlegm,shortness of breath in the observation group were lower than those in the control group and the differences were statistically significant（ P 〈0. 05）. The total effective rate of the observation group was91. 7% while that of the control group was 72. 2% and the difference had statistical significance（ χ^2= 4. 600,P =0. 032）. There were statistically significant differences between the observation group and the control group in physical,psychological,daily activities and social entertainment,sports,work and study and family（ P〈 0. 05） while the differences in the aspects of sex and the treatment had no statistical significance（ P〉 0. 05）. Conclusion： The clinical effect of Jingui Shenqi Decoction combined with Linggui Zhugan Decoction is good in the treatment of chronic obstructive pulmonary disease（ spleen- kidney Yang deficiency）,therefore it can be recommended in clinical application.
