益气养阴清肺法联合西药治疗初治肺结核的疗效及对免疫功能的影响研究

摘　　要：目的:探索益气养阴清肺法联合西药治疗初治肺结核的临床疗效及对患者免疫功能的影响。方法:纳入研究者为2012年12月—2014年8月临床确诊的76例初治肺结核患者,采用随机、单盲法进行分组,其中对照组38例患者均采取西药治疗,而观察组38例患者在西药治疗基础上加用益气养阴清肺方治疗,观察并比较对照组与观察组的临床治疗效果及患者CD+3、CD+4、CD+8和CD+4/CD+8T细胞亚群水平变化。结果:观察组患者经治疗后痰菌转阴率为92.11%,而对照组经治疗后痰菌转阴率为78.95%,观察组较对照组明显降低且经统计学分析组间差异显著(P<0.05)。观察组患者与对照组患者病灶吸收率和空洞闭合率比较均无显著性差异(P>0.05)。观察组患者经治疗后CD+3、CD+4和CD+4/CD+8比值分别为(65.33±6.78)、(47.63±5.76)、(1.84±0.75),均较治疗前和对照组治疗后明显增加,而CD8+水平为(22.87±2.11),较治疗前和对照组治疗后明显减少,经统计学分析显示观察组与对照组上述各指标差异显著(P<0.05)。结论:益气养阴清肺法联合西药治疗初治肺结核的临床疗效显著,且能明显改善患者免疫功能,有临床推广和应用价值。Objective： To explore the clinical effect of Yiqi Yangyin Qingfei method combined with western medicine in the initial treatment of pulmonary tuberculosis and its effect on immune function in patients. Methods： 76 cases of pulmonary tuberculosis in our hospital from December 2012 to August 2014 were divided into two groups by randomized,single blind method. 38 patients in control group were treated by western medicine treatment while the observation group＇s 38 patients in the western medicine treatment combined with Yiqi Yangyin Qingfei Decoction. The clinical effect and changes of CD3^＋,CD4^＋,CD8^＋and CD4^＋/ CD8^＋ T cell subsets levels were observed and compared. Results： The sputum negative conversion rate of the observation group was 92. 11% while that of the control group was 78. 95% and the observation group was significantly lower than that of the control group（ P〈 0. 05）. The focus absorption and cavity closure rate between two groups had no significant difference compared with the control group patients（ P 〉0. 05）. After treatment,CD3^＋,CD4^＋and CD4^＋/ CD8^＋ratio of the observation group were（ 65. 33 ± 6. 78）,（ 47. 63 ± 5. 76） and（ 1. 84 ± 0. 75）and were significantly increased compared with those before treatment and control group＇s after treatment,while the CD8^＋level was（ 22. 87 ± 2. 11） and significantly reduced compared with those before treatment and control group＇s after treatment（ P〈 0. 05）. Conclusion： The clinical effect of Yiqi Yangyin Qingfei method combined with western medicine in the treatment of pulmonary tuberculosis patients is effective and it can significantly improve the immune function of patients,with clinical promotion and application value.
