左氧氟沙星辅助抗结核药物治疗耐多药肺结核的临床疗效

摘　　要：目的探讨左氧氟沙星辅助抗结核药物治疗耐多药肺结核的临床疗效。方法选取2010年3月—2013年8月攀枝花市第四人民医院收治的耐多药肺结核患者140例,随机分为研究组与对照组,各70例。研究组患者予以左氧氟沙星辅助抗结核药物治疗,对照组患者予以常规抗结核药物治疗。观察两组患者治疗3、6、12个月及治疗后1个月痰菌转阴情况、影像学疗效(结核灶改善疗效、肺空洞改善疗效)及不良反应(胃肠道反应、肝功能轻度受损、肾功能轻度受损、头晕乏力)发生情况。结果研究组患者治疗3个月及治疗后1个月痰菌转阴率高于对照组,差异有统计学意义(P<0.05);两组患者治疗6、12个月痰菌转阴率比较,差异无统计学意义(P>0.05);研究组患者结核灶改善疗效和肺空洞改善疗效优于对照组,差异有统计学意义(P<0.05);两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论左氧氟沙星辅助抗结核药物治疗耐多药肺结核的临床疗效显著,可有效杀灭结核杆菌,且不良反应少。Objective To explore the effect of levofloxacin auxiliary of antitubercular agents in treating multi - drug resistant pulmonary tuberculosis. Methods A total of 140 patients with multi - drug resistant pulmonary tuberculosis were se-lected in the Fourth People＆#39;s Hospital of Panzhihua from March 2010 to August 2013,they were randomly divided into study group and control group,70 cases in each group. Study group were given levofloxacin auxiliary of antitubercular agents treatment, control group were given conventional antitubercular agents treatment. Treatment for 3,6,12 months and 1 month after treat-ment,sputum became negative rate,imaging effect（tuberculous foci improve efficacy,lung cavity improve efficacy）and inci-dence of adverse reactions（ gastrointestinal reaction,liver function damage,renal function damage,dizziness and hypody-namia）between the two groups were observed. Results Treatment for 3 months and 1 month after treatment,sputum became negative rate of study group were higher than those of control group（P ﹤ 0. 05）;treatment for 6 ,12 months,tsputum became negative rate showed no significant differences between the two groups（P ﹥ 0. 05）;tuberculous foci improve efficacy and lung cavity improve efficacy of study group were better than those of control group（P ﹤ 0. 05）;incidence of adverse reactions showed no significant differences between the two groups（P ﹥ 0. 05）. Conclusion Levofloxacin auxiliary of antitubercular agents have notable curative effect in the treatment of multi - drug resistant pulmonary,can effectively kill mycobacterium tuberculosis and has less adverse reactions.
