成人接种单剂乙型肝炎疫苗免疫应答及其影响因素分析

摘　　要：目的分析成人单剂乙型肝炎疫苗（HepB） 免疫应答效果及其影响因素.方法选择16～54岁成人1678人进行免疫前血清HBsAg、抗-HBs和抗-HBc定量检测,其中3项指标均阴性的980人给予60μgHepB1剂,并在免疫后1个月检测抗-HBs,分析抗-HBs阳转率和滴度水平.结果980人免疫后1个月抗-HBs阳转率为19.39%,几何平均滴度为3.42IU/L.多因素分析显示年龄是影响免疫应答的主要因素（P〈0.05）,抗-HBs阳转率以16岁～组最高,为88.46%,其次是25岁～组,为41.30%.二分类Logistic回归分析显示,16岁～、25岁～组的免疫应答效果好于其他年龄组（P〈0.01）.结论成人接种单剂60μgHepB免疫应答效果一般,在不同年龄人群中16～〈25岁组免疫效果良好.年龄越小,免疫应答效果越佳.Objective To evaluate the immunological effects of 1 dose of 60 μg hepatitis B vaccine among adults. Methods A Total of 1 678 adults aged 16 to 54 years were selected,and HBs Ag,anti- HBs,anti- HBc of them were detected.Among them,the HBs Ag,anti- HBs,anti- HBc of 980 adults were detected negative,and they were following 60 μg hepatitis B vaccine injection. The positive conversion rate and titer of anti- HBs of them were detected 1 month after the injection. Results The positive conversion rate was 19. 39%,and the GMT was 3. 42 IU / L at 1 month after immunization.Multivariate analysis showed that age was the influencing factor of immune response( P < 0. 05). The positive conversation rate was highest among 16- age group,followed by 25- age group. Binary logistic regression analysis showed that the immunological effects among 16- and 25- age groups were better than other age groups( P < 0. 01). Conclusions The effect of adults with single dose of 60 μg hepatitis B vaccine could not achieve the expected goal. Age is the influencing factors for immune response.
