65岁及以上老年人群HbA1c水平与血清CA19-9和CEA的关联性研究

摘　　要：目的探讨65岁及以上老年人群糖化血红蛋白（HbA1c）水平与血清糖类抗原19-9（CA19-9）、癌胚抗原（CEA）等肿瘤标记物的关系.方法对航天中心医院2012年参加健康体检的730名≥65岁老年人进行CA19-9、CEA、HbA1c和血脂检测.结果730名老年人中,HbA1c偏高占10.14%.单因素分析显示,HbA1c正常组（HbA1c〈7.0%）与偏高组（HbA1c≥7.0%） 在高甘油三酯血症、低高密度脂蛋白胆固醇血症、CA19-9和CEA等指标间差异均有统计学意义（P〈0.05）;多因素Logistic回归分析显示,高甘油三酯血症、低高密度脂蛋白胆固醇血症、CEA升高和CA19-9升高是血清HbA1c升高的影响因素,其OR值（90%CI） 分别为1.663（1.077～2.569）、2.202（1.296～3.741）、3.903（1.169～13.029） 和5.232（2.532～10.809）.结论65岁及以上老年人群血清HbA1c水平与CA19-9、CEA存在关联,血糖控制不佳的老年人群建议关注消化系统血清肿瘤标记物水平.Objective To explore the correlation between serum Hb1 Ac,CA19- 9 and CEA among old people aged 65 years or above,and to provide evidence for the target risk population that need tumor marker testing. Methods Information of 730 people aged 65 years or above were collected in Aerospace Center Hospital,and their venous blood sample were collected for testing CA19- 9,CEA,Hb A1 c and blood lipid. Results Serum Hb A1 c level of 74 people were elevated,accounting for 10. 14%. Results of comparison between Hb A1 c normal group( Hb1 Ac < 7. 0%) and elevated group( Hb1Ac≥7. 0%) showed that there were significant difference on hypertriglyceridemia,low high- density lipoprotein cholesterol( HDL- C), CA19- 9 and CEA( P < 0. 05). Multivariate logistic regression analysis showed that hypertriglyceridemia,low HDL- C,CEA,CA19- 9 were the influence factors of the high level of serum Hb A1 c,and the OR values were 1. 663( 90% CI: 1. 077- 2. 569),2. 202( 90% CI: 1. 296- 3. 741),3. 903( 90% CI: 1. 169-13. 029),5. 232( 90% CI: 2. 532- 10. 809),respectively. Conclusions Level of serum Hb A1 c is associated with serum CA19- 9 and CEA among the old people aged 65 years or above,and digestive system serum tumor markers were recommended to be tested among old people with poor blood glucose control condition.
