摘  要:目的探讨不同肥胖测量指标与血清胰高血糖素的相关性。方法选取68例2型糖尿病及67例非糖尿病病人,均进行标准馒头餐试验或口服75g葡萄糖耐量试验(OGTT),检测两组病人0、0.5、1.0、2.0h血清中的胰高血糖素水平,分析体质量指数(BMI)、腰围(WC)、腰臀比(WHR)和腰身比(WHtR)等肥胖测量指标与胰高血糖素的相关性。结果 0、0.5、1.0、2.0h时,糖尿病体质量超标准(超标)与肥胖组胰高血糖素水平显著高于糖尿病正常体质量组(H=9.83~16.17,P〈0.05);0、0.5、1.0h时,糖尿病超标组胰高血糖素水平显著高于非糖尿病超标组(U=174.00~295.00,P〈0.05);0、0.5、1.0、2.0h时,糖尿病腹型肥胖组的胰高血糖素水平显著高于非糖尿病腹型肥胖组(U=574.00~1 051.50,P〈0.05),亦显著高于糖尿病正常组(U=51.00~196.00,P〈0.05)。0、0.5、1.0、2.0h时,糖尿病WHtR≥0.52组胰高血糖素水平显著高于非糖尿病WHtR≥0.52组(U=542.00~907.00,P〈0.05),亦显著高于糖尿病WHtR〈0.52组(U=87.00~159.00,P〈0.05)。糖尿病组胰高血糖素水平与WC、BMI、WHR、WHtR均呈显著正相关(r=0.251~0.418,P〈0.05)。结论肥胖2型糖尿病病人存在高胰高血糖素血症。肥胖非糖尿病病人空腹时亦存在高胰高血糖素血症。控制体质量,尤其是控制腹型肥胖可能有助于肥胖者改善胰腺α细胞功能。Objective To investigate the correlation between obesity index and serum glucagon. Methods Sixty-eight patients with type 2diabetes and 67 patients with non-diabetes were enrolled and received either standardized steamed bread meal test or oral 75 g glucose tolerance test(OGTT).The serum levels of glucangon at fasting,and 0.5,1.0and 2.0hafter OGTT were measured.The correlation between glucangon level and BMI,WC,WHR and WHtR was analyzed. Results The results of glucagon levels- at 0h,0.5h,1.0h,and 2.0h-were as follows:In overweight or obese diabetic group,the levels of glucagon were higher than that in the diabetics of normal body weight(H=9.83-16.17,P〈0.05);The levels of glucagon in diabetic overweight group were higher than that in overweight but with no-diabetes group(U=174.00-295.00,P〈0.05);The levels of glucagon in diabetics of abdominal obesity group were higher than that in non-diabetics of abdominal obesity group(U=574.00-1051.50,P〈0.05),and also higher than diabetics of normal bodyweight group(U=51.00-196.00,P〈0.05).When WHtR ≥0.52,the levels of glucagon were higher in diabetic group versus non-diabetics(U=542.00-907.00,P〈0.05),and also higher than diabetic group of WHtR〈0.52group(U=87.00-159.00,P〈0.05).In diabetes group,glucagon level was positively correlated with BMI,WC,WHR,and WHtR(r=0.251-0.418,P〈0.05). Conclusion There is glucagonemia in obese adults with type 2diabetes,and also in obese patients with non-diabetes.Controlling body mass,abdominal obesity in particular,conduces to improving function of pancreatic alpha cells.
