IL-18启动子基因多态性与原因不明复发性流产关系

摘　　要：目的探讨白介素-18(IL-18)启动子基因-607C/A和-137G/C位点单核苷酸多态性(SNP)与中国汉族原因不明复发性流产(URSA)的相关性,并进一步分析其与血清IL-18水平的关系。方法采用特异性引物聚合酶链反应(PCR-SSP)技术,检测94例URSA病人(URSA组)和102例健康早孕妇女(对照组)IL-18启动子基因-607C/A和-137G/C位点基因型及等位基因频率;采用酶联免疫吸附法(ELISA)测定其血清IL-18含量。结果URSA组IL-18启动子基因-607C/A位点CC型频率高于对照组,差异有显著性(χ2=30.745,P<0.001),CA型频率低于对照组,差异有显著性(χ2=26.217,P<0.001),AA型频率低于对照组但差异无显著性(P>0.05);等位基因C频率高于对照组,差异有显著性(χ2=24.470,P<0.001)。IL-18启动子基因-137G/C位点基因型频率及等位基因频率两组比较差异均无显著性(P>0.05)。URSA组IL-18启动子基因-607C/A位点CC型者血清IL-18水平高于CA、AA型者,差异有显著性(t=43.056,P<0.001);URSA组IL-18启动子基因-137G/C位点3种基因型之间血清IL-18水平差异均无显著性(P>0.05)。结论 IL-18启动子基因-607C/A位点的SNP与中国汉族人群发生URSA的风险具有相关关系,它可能是通过影响IL-18基因表达而参与URSA的发病。Objective To investigate the association of IL-18 promoter gene-607C/A and-137G/C positions single nucleotide polymorphisms（SNP）with unknown recurrent spontaneous abortion（URSA）in Chinese Han nationality,and further analyze the relationship between the gene polymorphism with serum IL-18 levels. Methods Utilizing the PCR-SSP technique to detect the genotype of IL-18 promoter gene-607C/A and-137G/C positions in 94 patients with URSA and 102 healthy prepregnancy women.Serum IL-18 levels were detected using ELISA technique. Results The frequency of IL-18 promoter gene-607C/Aloci CC genotype in URSA group was significantly higher than in the control group（χ2=30.745,P〈0.001）,and the frequency of CA genotype was lower（χ2=26.217,P〈0.001）,the frequency of AA genotype was lower than that in the control group,but the difference was not statistically significant（P〉0.05）,the allelic frequency of C was significantly higher than that in the control group（χ2=24.470,P〈0.001）.The difference of the genotype and allele frequencies of the IL-18 promoter gene-137G/C positions between the two groups was not significant（P〉0.05）.In URSA group,the patients with IL-18 promoter gene-607C/A loci CC genotype had significantly higher serum IL-18 levels than those with CA and AA（t=43.056,P〈0.001）.There were no significant differences between GG,GC,and CC genotype of serum IL-18 levels at IL-18 promoter gene-137G/C positions. Conclusion IL-18 promoter gene-607C/A positions SNP is associated with Han patients with unknown origin recurrent spontaneous abortion in China,which is likely to involve in the occurrence of abortion through impacting the expression of IL-18 gene.
