摘 要:目的通过考察不同分期糖尿病肾病患者胃肠激素与糖尿病肾病的相关性,确定胃肠激素在糖尿病肾病患者胃肠动力障碍扮演的作用。方法选取2011年1月—2012年1月就诊的糖尿病肾病患者98例,患者按照DN分期分为DNⅠ期组(24例)、DNⅡ期组(26例)、DNⅢ期组(23例)和DNⅣ期组(25例),另选取同期进行健康体检的患者25例作为对照组,检测患者的血糖、胰岛素水平、糖化血红蛋白、促胃液素、胃动素、胰高血糖素和24h尿微量白蛋白排泄量,并进行统计分析。结果各组患者在病程、年龄、FBS、PBS和Hb A1c差异均无统计学意义(P>0.05);DNⅠ期组、DNⅡ期组、DNⅢ期组和DNⅣ期组促胃液素均显著高于对照组(P<0.01);DNⅠ期组、DNⅡ期组、DNⅢ期组和DNⅣ期组胃动素均显著高于对照组(P<0.01);DNⅠ期组、DNⅡ期组、DNⅢ期组和DNⅣ期组胰高血糖均显著高于对照组(P<0.01);促胃液素、胃动素、胰高血糖均和UAE正相关(P<0.05)。结论促胃液素、胃动素和胰高血糖与DN发病程度呈正相关,临床中可通过检测DN患者胃肠激素水平来确定胃肠动力障碍病变程度。Objective To investigate the correlation between the levels of gastrointestinal hormone and diabetic nephropathy(DN) in patients with different stages of DN, and evaluate the influence of the levels of gastrointestinal hormones on the gastrointestinal motility disorder in patients with DN. Methods Total 98 patients with diabetic nephropathy from in January 2011 to 2012 in January in our hospital were enroUed and randomly divided into DN I stage group(24 case), DN II stage group(26 case) ,DN III stage group(23 case) and DN IV stage group(25 case) ,and 25 individuals with health examination were recruited control group. The levels of blood glucose, insulin, glycated hemoglobin, gastrin, motilin pancreas, glucagon and 24 h-urinary albumin excretion were tested. The correlation between the levels of gastrin, motilin pancreas and 24 h-urinary albumin excretion was analyzed. The student t-test was conducted to analyze the difference in gastrointestinal hormone levels between different groups, Spearman correlation analysis (inspection) was used to analyze the correlation among the gastric juice, gastric dynamic element pancreatic, glueagon and 24 h urine trace albumin excretion. Results There has no statistically different between different groups in age, FBS, PBS and HbAlc. The levels of gastrin in DN I stage group, DN II stage group, DN III stage group and DN IV stage group were significantly higher than that in the control group(P 〈0.01 ) ;the levels of Motilin in DN I stage group,DN II stage group,DN III stage group and DN IV stage group were significantly higher than that in the control group ( P 〈 0.01 ) ; the levels of glucagon in DN I stage group, DN II stage group, DN III stage group and DN IV stage group were significantly higher than that in the control group( P 〈 0. 01 ).
