摘 要:目的探讨绞窄性肠梗阻大鼠肠黏膜免疫屏障功能变化及胸腺肽α1对其影响。方法将48只雄性SD大鼠随机分为假手术组(A组,8只)、对照组(B组,20只)、实验组(C组,20只)。A、B组大鼠每日给予生理盐水0.1mL皮下注射,C组大鼠每日给予胸腺肽α1 0.16mg皮下注射。注射第4天B、C组大鼠行末端回肠和肠系膜血管结扎术制备绞窄性肠梗阻大鼠模型。术后各组大鼠继续给予生理盐水或胸腺肽α1皮下注射。术后72h,采用酶联免疫吸附法检测动脉血血清内毒素和盲肠肠黏膜组织sIgA含量,免疫组化染色法计数回肠末端黏膜固有层CD3^+、CD4^+、CD8^+T细胞百分数。结果与A组比较,B组大鼠回肠末端肠黏膜CD3^+、CD4^+、CD8^+T细胞百分数和盲肠肠黏膜组织sIgA含量明显降低,血清内毒素含量明显升高(F=6.84~17.66,t=3.49~5.94,P〈0.05)。与B组相比,C组大鼠血清内毒素含量明显降低(F=12.59,t=2.74,P〈0.05),回肠末端肠黏膜CD3^+、CD4^+、CD8^+T细胞百分数明显升高(F=6.84~17.66,t=2.66~2.99,P〈0.05),盲肠肠黏膜组织sIgA含量升高,但差异无显著性(P〉0.05)。结论绞窄性肠梗阻大鼠的肠黏膜免疫功能下降,肠道内毒素移位增加。胸腺肽α1可以改善绞窄性肠梗阻大鼠的肠黏膜免疫功能,减少肠道内毒素移位的发生。Objective To investigate the influence of thymosin-α1on changes of intestinal mucous membrane immunologic barrier in rats with strangulated intestinal obstruction(SIO). Methods Forty-eight male SD rats were divided into three groups in random:group A(sham-operation group,n=8),group B(SIO group,n=20)and group C(thymosin-α1group,n=20).The rats in groups A and B were given subcutaneous injection of normal saline 0.1mL,once a day;those in group C were given subcutaneous injection of thymosin-α1 0.16 mg,once a day.On day 4of the injections,the rats in groups B and C received ligation of terminal ileum and mesentery blood vessels to create a model of SIO.After surgery,normal saline or thymosin-α1 0.16 mg was continued as before.Rats were sacrificed 72 hours after creation of the models.Serum endotoxin in arterial blood and sIgA in mucous membrane of cecum were detected using enzyme-linked immunosorbent assay.The percentages of CD3^+,CD4^+,and CD8^+T lymphocytes in intestinal mucosal lamina propria of terminal ileum were counted using immunohistochemistry method.Results Compared with group A,the percentages of CD3^+,CD4^+,and CD8^+T lymphocytes in intestinal mucosal lamina propria of terminal ileum and content of sIgA in mucosa of caecum in group B were lower,and serum endotoxin was higher(F=6.84-17.66,t=3.49-5.94,P〈0.05).Compared with group B,the level of serum endotoxin in group C was lower(F=12.59,t=2.74,P〈0.05),the percentages of CD3^+,CD4^+,and CD8^+T lymphocytes in mucous membrane of terminal ileum were higher,the differences were significant(F=6.84-17.66,t=2.66-2.99,P〈0.05),the sIgA content in mucous membrane of caecum was elevated,but the difference was not statistically significant(P〉0.05). Conclusion In rats with strangulated intestinal obstruction,their intestinal immune function descend,and displacement of intestinal endotoxin increases.Thymosin-α1can improve the intestinal immune function and reduce endotoxin translocation in strangulated inte
