摘 要:目的探讨退变性腰椎侧弯伴椎管狭窄症神经根性痛与腰椎侧弯的关系。方法2009年1月—2014年1月,我院收治退变性腰椎侧弯伴椎管狭窄症病人100例,通过疼痛分布区、神经学及影像学检查确定受累神经根的受压部位,并分析其与退变性腰椎侧弯的关系。结果 L4神经根多在侧弯的凹侧受压,L5神经根多在侧弯的凸侧受压(χ2=25.118,P<0.001);L4、L5神经根受压明显多于L3、S1神经根(χ2=109.107,P<0.001)。症状与主弯及代偿弯无关(P>0.05)。结论退变性腰椎侧弯伴椎管狭窄症病人的神经根受压情况复杂多变,L4神经根多在凹侧受压,L5神经根多在凸侧受压,L4、L5神经根受压明显多于L3、S1神经根。Objective To explore the relationship between nerve root pain and lumbar lateral curvature(LLC)in degenerative lumbar scoliosis(DLS). Methods From January 2009 to January 2014,100 patients with DLS complicating spinal stenosis were treated in our hospital.Through examination of pain distribution,neurology and imaging to determine the part of compressed nerve root,the association of it with LLC was analyzed. Results L4 nerve root compression mostly located in concave side of scoliosis,and L5 nerve root mostly in convex side of scoliosis(χ^2=25.118,P〈0.001),L4 and L5nerve roots were more compressed than L3 and S1(χ^2=109.107,P〈0.001).The symptoms were not associated with main scoliosis and compensatory scoliosis(P〉0.05). Conclusion The condition of nerve root compression in patients with degenerative lumbar scoliosis accompanying spinal stenosis is complex and changeable,L4 is mostly compressed in concave side,L5 is commonly seen in convex side,L4 and L5are more markedly involved than L3 and S1do.
