黄芪桂枝五物加水蛭汤联合西药治疗脑梗死随机平行对照研究

摘　　要：[目的]观察黄芪桂枝五物加水蛭汤联合西药治疗脑梗死疗效。[方法]使用随机平行对照方法,将121例住院患者按病志号抽签方法简单随机分两组。对照组54例1稳定血压血糖,控制脑水肿、体温、基础疾病及合并症,加强护理;2溶栓治疗时间窗（发病1~6h）内无禁忌症入院用尿激酶100万单位静脉注射,24h后口服阿司匹林300mg（阿司匹林过敏史或消化性溃疡改氯吡格雷75mg口服,1次/d）;3降血脂、营养神经、早期康复。治疗组67例黄芪桂枝五物加水蛭汤,黄芪50g,赤芍,桂枝、生姜各12g,大枣10g,水蛭9g,1剂/次,开水150m L冲服,2次/d;西药治疗同对照组。连续治疗30d为1疗程。观测临床症状、神经功能缺损积分、满意率、不良反应。连续治疗3疗程,判定疗效。[结果]临床疗效两组间无明显差异（P〉0.05）。满意率治疗组优于对照组（P〈0.05）。不良反应两组间无明显差异（P〉0.05）。[结论]黄芪桂枝五物加水蛭汤联合西药治疗脑梗死,疗效满意,提高满意率,值得推广。[Objective]To observe the five drus plus leech decoction combined with western medicine treatment of cerebral infarction.[Methods]121 cases of patients were randomly divided into two groups according to the patients with the disease.Control group of 54 cases.1To stabilize blood pressure,control the intracranial hypertension cerebral edema,control body temperature,treatment of basic diseases and complications,strengthen nursing;2In the time window of thrombolytic therapy（incidence of 1~6h） within and without contraindications were available for 1000000 units of urokinase for intravenous injection,24 h after oral administration of aspirin 300mg（aspirin allergy history or peptic ulcer were treated with clopidogrel 75 mg oral administration,QD）;3Lowering blood lipids,nutrition,early rehabilitation.The treatment group of 67 cases five drus plus leech decoction,Astragalus 50 g,radix paeoniae rubra,cassia twig,ginger,jujube 12 g,Jujube 10 g,Leech 9g,1 agent/times,2 times with boiling water 150 m L,BID;Western medicine treatment with control group.30 d is a continuous treatment for 1 courses.Observation of clinical symptoms,neurological deficit scores,satisfaction rate,adverse reactions.Continuous treatment of 3 courses,determine the efficacy.[Result]There was no significant difference in clinical efficacy between the two groups（P〉0.05）.The satisfactory rate of the treatment group was better than that of the control group（P〈0.05）.There was no significant difference between the two groups（P〉0.05）.[Conclusion]Huangqi Guizhi five plus leech decoction combined with western medicine treatment of cerebral infarction,satisfactory curative effect,improve the satisfaction rate,worthy of promotion.
