地黄汤联合碳酸氢钠治疗痛风性肾病随机平行对照研究

摘　　要：[目的]观察地黄汤联合碳酸氢钠治疗痛风性肾病疗效。[方法]使用随机平行对照方法,将80例门诊患者按抛硬币法简单随机分为两组;低嘌呤食物为主,严格控制蛋白质摄入量;禁酒,多饮水,碳酸氢钠,1g/次,2次/d。对照组40例别嘌呤醇,300mg/次,2次/d。治疗组40例地黄汤(生地黄30g,山萸肉、怀山药各15g,丹皮10g,怀牛膝、泽泻各15g,土茯苓、玉米须各30g,秦艽15g,金钱草30g,海桐皮10g,豨莶草15g),1剂/d,水煎300m L,饭后口服,3次/d,100m L/次。连续治疗60d为1疗程。观测临床症状、血液生化指标(血肌酐、血尿酸、血清2-微球蛋白、尿素氮、尿蛋白)、不良反应。治疗1疗程,判定疗效。[结果]治疗组显效23例,有效15例,无效2例,总有效率95.00%。对照组显效12例,有效20例,无效8例,总有效率80.00%。治疗组疗效优于对照组(P<0.05)。血液生化指标两组均有改善(P<0.05),治疗组改善优于对照组(P<0.05)。[结论]地黄汤联合碳酸氢钠治疗痛风性肾病疗效满意,无严重不良反应,值得推广。[Objective]To observe Huang Tang combined with sodium bicarbonate gouty nephropathy.[Methods]randomized,parallel controlled method,80 outpatients law simply by flipping a coin randomly divided into two groups.In low-purine foods,strict control of protein intake;alcohol,more water,sodium bicarbonate,1g/times,2 times/d.The control group 40 cases Allopurinol,300mg/times,2 times/d.Treatment group,40 patients Dihuang soup（Shengdihaung 30 g,Shanyurou,Huaishanyao each 15 g,Danpi 10 g,Huainiuxi,Zexie 15 g,Tufuling,Yumixu each 30 g,Qinjiu 15 g,Jinqiancao 30 g,Haitongpi 10 g,Xixiancao 15g）,1/d,decoction 300 m L,meal orally,3 times/d,100 m L/times.60 d for a course of continuous treatment.Observation of clinical symptoms,blood biochemical parameters（creatinine,uric acid,serum 2-microglobulin,urea nitrogen,urine protein）,adverse reactions.One course of treatment,to determine efficacy.[Results]The treatment group 23 cases,effective in 15 cases,2 cases,the total efficiency of 95.00%.The control group 12 cases markedly effective in 20 cases,8 cases,the total efficiency 80.00%.Treatment group than the control group（P〈0.05）.Blood biochemical parameters improved in both groups（P〈0.05）,treatment group was better than that in the control group（P〈0.05）.[Conclusion]Dihuang soup combined with sodium bicarbonate treatment of gouty nephropathy satisfactory,no side effects,worthy of promotion.
